PARENT’S CONSENT FORM

Dear parents and guardians,

Adelante greetings!

The Senior High School club members of the Graphics, Arts, and Technicalities will
participate in the Club Activity Day and Community Outreach Program in Barangay
Basak Pardo Day Care Center on March 06, 2024, Wednesday from 1:00 pm - 4:00 pm.

With this, we would like to request your permission to allow your child or ward to attend
the activity on the said date. Additionally, transportation from the school to the venue
and vice versa will be provided and they will be accompanied by their club advisers.
Assembly time is 12:00 pm in the Brgy. Basak Pardo, Barangay Hall, and we will depart
at 12:30 pm.
