INFORMATION AND CONSENT

FORM By participating in Altered Stories on 5/08/2024 __

I agree and consent to this legally binding Agreement with Altered
Stories Ministries (Altered Stories) and declare that I have full
authority and capacity to sign below relating to this consent.

I grant Altered Stories the rights to record and have recorded my

voice, likeness, image and/or information provided by me
("Recording/s"). This Agreement applies to any and all recordings
taking place within the event described above.

I grant Altered Stories unlimited and unrestricted rights, worldwide, to

publish, republish, transcribe, print, reprint, exhibit, broadcast,
rebroadcast, transmit, retransmit, and/or distribute recordings in any
form, in whole and/or in part, edited and/or unedited, with and/or
without associated editorial comments, in any and all languages, and
in any and all media and formats now known or hereinafter devised;
the right to translate into any language any and all of the foregoing;
and the right to sublicense and/or assign any and all of the foregoing
rights. I make said grant for the life of the copyright/s in the
Recording/s, and waive any and all right/s to receive fees and/or
royalties related to the Recording/s.

I forever waive any and all rights and claims I might otherwise have
against Altered Stories arising from or relating to the rights granted
herein, including but not limited to injunctive and/or other equitable
relief; defamation; invasion of privacy; blurring and distortion;
tortuous injury; and infringement of the right of publicity.

The parties agree that this transaction may be conducted by

electronic means using an electronic signature.
