ATTENDANCE POLICY

GENERAL

Excellent attendance is an expectation of all employees of Company Name. Daily


attendance is especially important for hourly employees whose clients have the expectation
of on-time, noted progress.

CALL IN PROCEDURE

Any employee calling in sick must do so by contacting their immediate supervisor no later
than 60 minutes prior to start of work day.

Should an employee be out sick/injured for more than 2 consecutive days, a doctor’s note
declaring the employee ‘fit for return to work’ must be presented prior to any work
performed. This note must be dated within the time frame of reported illness and presented
to Company’s Name Safety Advisor before employee resumes work.

Any employee of Company Name must notify their Foreman and Safety Advisor
immediately if they are calling in sick due to work related pain.

FAILURE TO CALL or AWOL (Absent With Out Leave)

Failure to call in prior to your shift start will result in activating Company’s Name
Progressive Discipline Policy.

First Offence – Verbal Warning (recorded)


Second Offence – Written Warning (and potential suspension without pay-recorded)
Third Offence – Dismissal

Absent for more than 2 consecutive days, Company Name. will consider this event as a
voluntary resignation and are therefore released from Company’s Name. employ.

CONSEQUENCES OF TARDY, ABSENCE and ABUSE of EMERGENCY – PERSONAL


TIME

The accumulation of eight tardies or absences in a “rolling” year (any consecutive twelve
months) is grounds for employment termination. Disciplinary action, that may lead up to
and include employment termination. Action may begin when the third tardy/absence in a
three month time period is recorded.

Anytime an employee is late for shift or has an unauthorized absence, his/her time sheet
and Foreman’s Report must reflect that.
Falsification of records (time sheets, foreman’s report and the like) will result in disciplinary
action up to and including dismissal regardless of position with Company Name.

EMERGENCY – SICK - PERSONAL TIME

Unpaid emergency personal time may be required by employees for such unscheduled
events as personal illness, immediate family member illness, and doctor appointments.

Employees who require emergency personal time must call and talk with their immediate
supervisor as soon as possible, but no later than sixty minutes prior to the start of their shift.
If the supervisor is unavailable, employees may leave a message for the supervisor with a
phone number where they can be reached. The supervisor will then return their call.

Overuse of emergency personal time or absence without valid reasons (validity determined
by Supervisor and Manager) will also result in disciplinary action up to dismissal.

On a case by case basis, within the normal schedule, the employee may make up the time
missed during the week in which the time was missed. Employees must have the
permission of their Manager to make up time if this option is available. An employee that
missed time due to work related injury is not permitted to make up time while on light
duties.

I have read and been informed about the content, requirements, and expectations of the
attendance policy for Company Name. I have received a copy of the policy and agree to
abide by the policy guidelines as a condition of my employment at Company Name. I
understand that if I have questions, at any time, regarding the attendance policy, I will
consult with my immediate Supervisor first, Manager and/or Payroll.
