PARENT CONSENT AND WAIVER FORM

I, ___________________, hereby give my consent and approval to this form by allowing my

daughter/son, ___________________ of BS Entrepreneurship, SBENT-1B to join the
community immersion at [place of barangay] every Monday, 3:00 PM to 6:00 PM, starting on
February 27, 2023 up until May 8, 2023. This activity is part of the requirements of the subject,
Civic Welfare Training Service 2 under National Service Training Program (NSTP) at the
Quezon City University.

We are aware that the immediate supervisor of the QCU-NSTP will take all the necessary
precautions to keep the student safe. NSTP-CWTS instructor, Jovelyn D. Encinas will be
present during community immersion to monitor the performance and safety of the students.

I, as parent/legal guardian, remain legally responsible for any personal action taken by the
student stated above. I have read and understand the terms of this consent and waiver.
