CORRECTIVE/PREVENTIVE ACTION FORMAT FOR PT

Department: Radiological Dept.

Corrective Action No. & Date: 20th Nov, 2023
Nodal CAB/Provider: IAEA
Sample Name: Water
Date of Sample Received: 20th Oct, 2023
Date of Sample Analysis: 4th and 5th Sept, 2023
Evaluation of Proficiency Test Results: The result of PT are quite satisfactory and upto mark and we have passed in
those Radionuclides Parameters also which does not come under our scope. The results in Alpha and Beta are quite
good. Except Cs-137, all reported parameters results are up to mark.

Root Cause Analysis: Cs-137 has gamma energy of 662 keV and the PT item has Cs-134 as well as Sb-125
radionuclides. These two radionuclides also have gamma energy of 600 and 620 keV respectively which will interfere
with the Cs-137 energy level. Our Gamma Spectrometer is based on NaI(Tl) Detector can’t resolve all three elements
peaks at once differently. Therefore Cs-137 has higher value then target value considering the acceptance of Cs-134
and Co-60. The performance can be considered as satisfactory.

Corrective Action Taken: We will participate in future PT

Preventive Action Taken: We have participated in MSME ILC and scored a Z Score of -0.88
