AFFIDAVIT UPON TURN-OVER ON

PURCHASED HOUSE AND LOT

I, ___________________________, Filipino, of legal age and with address at _______________________________, after

having been duly sworn to in accordance with law hereby depose and state that:

1. I purchased a house and lot from Peakland Prop. Inc. (the “Corporation”) specifically:
Subdivision : WESTDALE VILLAS

House Model : _______________
Location : Brgy. Punta, Tanza, Cavite
Payment Scheme : _______________
Date of Approval/Takeout : _______________
Turn Over Date : _______________
2. During orientation, I was informed by the Developer, and I understand and accept the following warranty coverage of said unit,
to wit:
a. In case of leakage and/or hole on GI roofing sheets: One (1) Year from acceptance; or during rainy season of the first
year of acceptance, whichever comes first;
b. Thirty (30) days only for the following:
i. Wall: Paint bubbles, seepages and cracks. Temperate or hairline cracks are NOT included;
ii. Water Line/Plumbing: Leaks;
iii. Electrical Line: short circuit;
iv. Sewer Line: clogging.
3. I fully understand that the foregoing coverage will only be valid provided that I introduce or make no improvement/s on the
unit/structure/building.
4. Moreover, I understand and accept that on the turnover date of the housing unit, my inspection shall only be limited to what
was written on my punch list (if there is any).

5. Those not included or made subject of concerns during the punch list inspection are automatically waived for any and/or future
complaints and I take full responsibility thereto.
6. I was informed and I accept that if I did not report/visit on the advised scheduled turn over date, the same shall automatically
be construed as constructive turnover (“as is where is”).
7. Through this Affidavit, I hereby fully release the Corporation, its shareholders, officers, directors and employees for any and all
damages which may be found beyond the above-provided coverage period.
8. Finally, I hereby take full responsibility to the security of the unit from the moment of acceptance or turnover, and likewise
hereby release the Corporation, its shareholders, officers, directors and employees for any and all claims for damages or
missing items from said date.
9. That I am executing this Affidavit to attest to the truth and veracity of the foregoing and for whatever legal purposes this
Affidavit may otherwise serve.
