I am sending this letter, on behalf of ALICIA’S RESPONSIBLE INDIVIDUAL FOR BETTER

ADOLESCENTS HEALTH, to convey our official intent to apply for local registration for youth
organizations and youth serving organizations in Bohol. We have read the local registration guide and thereby
submit the organization to the necessary processes and procedures as prescribed by Republic Act No. 10742.

Subsequently, please find in the attachment the following documents: (1) Registration Form, (2) Directory of
Officers and Advisers, (3) List of Members, (4) Constitution and By-Laws, and (5) state if the supporting
document is the Certification of Existence of Office or Barangay Certification of Residence or SK
Resolution.
