PARENT’S CONSENT FORM

This is to certify that I am allowing AMY-MAE A. ABYA to go to an internship for 45 weeks

from FERBUARY 19 to MAY 30 at TUMAUINI WEST CENTRAL SCHOOL in partial
fulfillment of the requirements for the degree in Bachelor of Secondary Education.

I also allow AMY-MAE A. ABAYA to go on fieldwork if the job assigned requires such.

It is understood that AMY-MAE ABAYA will follow the policies and guidelines set by the
school and abide by the rules and regulations that may be imposed by the school head/assigned
cooperating teacher for his/her welfare and safety.

I fully agree to waive any responsibility on the part of HGBaquiran College, Inc., Name of
cooperating school, and the school head/assigned cooperating teacher in case of any untoward
incident that may happen to AMY-MAE ABAYA in the duration of the teaching internship.
