KNOWLEDGE, ATTITUDE, AND PRACTICES OF RADIOLOGIC TECHNOLOGIST IN

IMAGING EQUIPMENT MAINTENANCE

(QUESTIONNAIRE)

You are invited to participate in a research study knowledge attitude and


practices of radiologic technologist in imaging equipment safekeeping, this research is
being conducted by Kench Bryan Magdaluyo and James Quirante Jr 3 rd Year College,
BSRT (Bachelor of Science in Radiologic Technology) students at Kidapawan Doctor’s
College Inc. The study addresses a few issues and seeks to shed light on radiologic
technologists' knowledge, attitude, and practices about the maintenance of imaging
equipment. The information will be used to complete this study and will be kept
confidential.

Dear Participants,
We are the 3rd year college students from the Radiologic Technology Department
of KDCI conducting our research regarding the knowledge, attitude and practices of
radiologic technologists in imaging equipment maintenance among selected hospitals in
Kidapawan City. This questionnaire will take 5-8 minutes to answer. There is no wrong
or wright answer on this questionnaire, thus please answer them with honesty. All
information will be held with utmost confidentiality.

Part II. Level of knowledge, attitude and practices of Radiologic Technologists in


imaging equipment maintenance
Instructions: Indicate your answers by checking the boxes below. Participation is
voluntary. Any questions may be skipped that may make you feel uncomfortable in
answering. Completion and return of this survey imply consent.
Directions: Please read each statement about equipment maintenance. Check (ü) the
box “true” if the statement is true, check “false” if the statement is false. If you don’t
know the answer, simply check the “I don’t know” box.

equipment is carried out every
day to identify any loose
components or vibration during
operation.
12. Lack of regular maintenance
of X-ray machine can increase
the risk of radiation leakages.
