Incident details
Describe in detail what happened – what, where, who, when, why and how. Include diagrams images and use additional pages if required

1.What happened?

Satma Subcontractor’s (landscaping) Skid steer loader 6646 caught fire while shifting soil/sand inside the villa 274 zone 3

Except the skid steer loader, no any other property damage or injury witnessed

2. Where happened?

Yas acres phase 3 Project is divided into two cluster E&F. This skid steer fire incident occurred in Cluster E Zone 3 villa 274
compound.

3.When did it happen?

On Friday, 5th April 2024 at about 11:30 AM.

4.Who was involved?


Skid steer operator who was shifting soil/sand inside the villa 274 Zone 3.

5.Why did it happen?


As per the investigation conducted with the skid steer loader’s operator named Yasir Ali and evidence collected from the incident
location might be following factor caused fire.
Such as,
Electrical short circuit.
Oil leakage because the fire was started from the engine of the skid steer loader and there are possibilities that oil was leaking and
caused fire
Unceasingly operation of skid steer loader which caused to enhance the heat in skid steer loader and result in a fire.

Note- above are the possible causes of fire ignition. Operator daily checklist was completed and as per the operator checklist it was
inspected and at the time of inspection he found satisfactory.

Immediate action-
Skid steer operator Yasir Ali as he feels the fire. He switched off engine and come out. He tried to extinguish the fire by using fire
extinguishers. HSE team also reached on the incident location and tried to extinguish the fire. Fire extinguished by using the fire
extinguishers, water and sand.
Abu Dhabi Civil Defence and police were called. However, the fire was extinguished before they reach.

Corrective action.
Fire extinguished immediately and restrain to spread around.
incident area isolated to stop unauthorized entry.
Immediately initial information disseminated with senior management and consultant etc.
Satma’s senior management also called at the incident location.

Preventive action-
All the plant and equipment shall be inspected regularly to identify any issues which may pose the fire or other risk.
Regular maintenance of equipment shall be established and recorded
If any observation witnessed equipment will be stopped until its rectifications.
Daily operator checklist shall be completed and ensured operator is filling the checklist after doing adequate inspection of
equipment.
Operators are advised to report any leakage or damage.

Short Description about the police and Civil Defence-

Civil Defence came on the site along with AD police. Civil Defence crew inspected the effected skid steer loader and disconnected
the battery connection and declared no further chance of fire ignite. Bob cat owner agreed with the police for no any claim or
liability on the Trojan or project etc.
After the complete investigation and police approval skid steer loader removed from the site.
