DIAGNOSIS-CEREBRAL PALSY
INTRODUCTION
Shanmuk is calm and quiet child. He is 12 years old. He is
diagnosed with CEREBRAL PALSY. He is able to speak
Hindi, Telugu and Tamil. His speech is not clear. He is very
co-operative boy. He is very much interest in participating
all activities and the efforts he puts in all skills.

PERSONAL/ADDL SKILLS: Shanmuk is a quiet child. He is a

social child ,he greets everyone who comes to school and
says bye while going home. He follows every instructions
given by the teacher for an activity.
SOCIAL SKILLS : Shanmuk is a co-operative boy .He
showing interest to interact with other students of the
class. He is able to speak Hindi ,English and Tamil
languages.
ACADEMIC SKILLS :Language: He can write three letter
words in English and Hindi. He also can write names of
his family members, days and months.
Mathematics: Shanmuk is practicing addition and
substraction .He can identify currency notes(10,Rs 20,Rs
50,Rs 100,Rs 200) and coins (1rs,2rs,5rs)
Time concept: He can read only hours in analog clock .
Concept : He is very good in academics .He have some
problems in writing due to gripping problem.
OCCUPATIONAL SKILLS
Shanmuk is trying to do every activities. He can fold
papers with the physical prompt.
MOTOR SKILLS
(a) Gross motor: Taking physiotherapy, Aiming to
make him walk independently. He participated in
Annual sports day . He wons 2 medals in buddy
race and links. He is active in games period at
school and try to play almost every game
 (b) Fine motor : He cant do much as has less
strength in fingers, poor gripping. We are trying to
help him improve FSM by making him do finger
strengthening activities.
RECREATION :
(a) Indoor: He loves to play ludo board and he can
solve simple puzzles. He is also interested in
painting and drawing.
(b) Outdoor: He participated in Annual sports day
and won buddy race and links. He participated
in weekly sports periods which includes passing
ball, frizz bee and musical chairs etc..
GENERAL REMARKS: Shanmuk has shown lot of
improvements in school. He is now interact and co-
operate with each and every student and teachers as
well. He interest in participating in every physical activity
and the efforts he puts in all skills. This academic session
he participated in Christmas, Holicelebration and Annual
sports day . He got first prize in two events in annual
sports day. He has shown little improvement in
academics. He is a child who has got lot of scope for
development with co-operation of teachers and parents.
