II. Program Background and Rationale
DepEd fully supports the continuing professional development of

its teaching personnel based on the principle of lifelong learning and
DepEd’s commitment to the development of teachers’ potential aimed
towards their success in the profession. This can be done through the
school-based LAC, which primarily functions as a professional learning
community for teachers that will help them improve practice and learners’
achievement.
Cognizant to the Department of Education Memorandum DM-
CI-2020-000 entitled- Clarification on the Use of the Most Essential
Learning Competencies (MELCs), paragraph 4- states that teachers shall
be tasked to prepare Learning Activity Sheets. In support of this
undertaking, schools are encouraged to develop activity sheets intended
for remediation, reinforcement, and advancement as school-developed
supplementary materials of division-developed modules to address the
needs and learning gaps of all types of learners. Thus, Namuco
Elementary School adheres on the policy to support the continuing
professional development of its teaching personnel by crafting different
type learning resources in all subject areas.
III. Program Description
The Project CREATE (Crafting Learning Resources: Evidence of

Accurate Test and Evaluation) focused on creating a localized and
contextualized learning materials. The said learning materials will be
evaluated through the Criteria in Evaluating Learning Resources (LR) and
some important points on how to assess the different types of LR.

The program participants are all 10 teaching personnel, and 1 non-

teaching.

Program participants apply their newly acquired competencies back

on the field in which the lessons learned helps them in coping with
some challenges that may arise in an actual job situation. This session
opens an opportunity to all (teaching and non- teaching personnel) to
develop their potentials relative to their profession.
VIII. Detailed Activity Implementation Plan
The School LAC Session will start at 3:00 o clock in the afternoon
and is divided three session which will be facilitated by Mrs. Lerma C.
Magnaye ( Head Teacher III), Leslie G. Tumambing (Teacher I) and
Shenna C. Amihan (Teacher III). The session will last up to 5:00 o
clock in the afternoon.

IX. Monitoring and Evaluation Plan
The person assigned to QAME ensures that the QAME process

such data gathering throughout monitoring tools and FGD analysis of
data as well as presentation of findings are observed and ensures
protocol is observed in the process. Consolidates QAME results and
submits report to the program manager/training manager. Overseas
the entire QAME process of the program.
