Level of validity of the Localized Supplementary Reading Material

Instructions: Examine the material carefully and for each evaluation criterion consider the extent to
which the resource meets the criteria. Check the appropriate number [with 5 being Very Highly
Acceptable (VHA); 4 - Highly Acceptable (HA); 3 - Moderately Acceptable (MA); 2 Slightly Not
Acceptable (SNA), and 1 – Not Acceptable (NA)]. For a rating below 4, write your
comments/justifications on each evaluation criterion. Attach extra sheets if necessary. Please write
legibly if completing in hardcopy
