AFFIDAVIT OF LOSS

I, VANBOYD CASANOS TORRES, 58 years old, married,

and a resident of Zone 4, Sinaloc, El Salvador City, Misamis
Oriental, Philippines, after having been duly sworn to an oath
in accordance with the law, hereby depose and say:

1. That I am the owner of a Globe simcard with number

0936-321-0354, which was already registered under my
name;
2. That I lost my cellular phone, with the said simcard on it,
on or about 1:00am of April 18, 2024 when I participated
as Press Witness during a buy bust operation at El
Salvador City, Misamis Oriental;
3. That despite due diligence and earnest efforts to locate it,
the same proved futile;
4. That I am executing this Affidavit to inform Globe Telecom
that my simcard was lost and for whatever legal purposes
it may serve; and
5. That I attest to the truth of the foregoing facts.

FURTHER AFFIANT SAYETH NONE.
