AFFIDAVIT OF UNDERTAKING AND COMMITMENT

I, CHARLIE A. DAQUIOAG, of legal age, single, and with postal address in Bangag,
Solana, Cagayan, 3503 hereby acknowledge and attest to the truthfulness of the
following:

1. I am a Second (2nd) year Criminology student at the University of Cagayan Valley

(UCV);

2. I am aware of the policy of the the University on the wearing of Identification (ID)
cards;

3. That I was not able to pay my assessment fees on time due to limited financial
resources;

4. That due to the situation mentioned in item No. 3, I purposely and intentionally
made a dummy ID to be able to enter the school premises pending the payment of my
assessment fees;

5. I was caught by the school guards using my dummy ID made out of printer paper;
and

6. I was ordered by the student affairs to execute this affidavit as a warning that any
future violation hereof, will cause more serious penalties or sanctions subject to proper
grievance procedures.

I further manifest and commit that I am disposing my dummy ID to express my

remorse and willingness to abide by the school policies.

Finally, I have read this document titled: the AFFIDAVIT OF UNDERTAKING and
COMMITMENT and fully understood every word it and its meaning and have affixed
my signature hereunder voluntarily and freely with the full and complete knowledge of
the meaning and intent of this document and of my rights under existing laws.

Espiritu St. Purok 1, San Pascual, San Narciso, Zambales hereby acknowledge and
attest to the truthfulness of the following:

1. I am a Second (2nd) year Criminology student at the University of Cagayan Valley

(UCV);
