AFFIDAVIT

WE, MANABI CHOUHAN, wife of Tushar Banerjee, by faith Hindu, by

Occupation:- Self employed, residing at Sorojini Apartment, 4/C, Rishi
Aurobindo Road, Madhyamgram Bazar, Sodepure Road P.O & P.S-
Madhyamgram, Pin no-700130, District:- North 24-Parganas AND
TUSHAR BANERJEE son of Late Prasanta Banerjee, by faith Hindu, by
occupation – Advocate, residing at 43, Ram Sita Ghat Street, P.O.-
Bhadrakali, P.S.- Uttarpara, District Hooghly, PIN- 712258, do hereby declare
and say as follows :

1) That our marriage was solemnized as per Hindu Rights

and customs on 1st day of March ,2015.
2) That out of the said Wed lock one male child namely Titas
Banerjee was born on 22/03/2016 . (A Xerox copy of Birth Certificate
is enclosed herewith).
3) That owing to difference in temperament , habits , thoughts
culture , opinion and increasing compatibility ,the relationship between
the Deponent No.1, and Deponent No.2 deteriorated and finally
Deponent no.1 left the matrimonial house 0n 12/02/2019.
4) That since then Deponent No.- 1 lived separately with her
parents ‘ house along with our minor child Titas Banerjee.

5) It is mutually agreed between us that custody of our minor child remain to the Deponent
No.-1 provided that Deponent No.-2 shall have the right to bring our minor child at his place
once in a month for at least 1 day ( 24 hours ).

6) It is further mutually agreed between us that we shall file mutual Divorce to competent
Court under Section 13(b) of the Hindu MARRIAGE ACT, 1954 as early as
possible.

7) That the purpose of swearing in this affidavit is to submit before the Competent Authority to
avoid any dispute in respect of our child custody.
