Circular: 13 / 2017: Crew fatal incident during cargo tank squeezing

Dear Captains,

Regret to inform that one of our crew fell in the cargo tank while coming up the ladder
upon completion of tank (8P) squeezing operation around 1900 LT on 12th Nov 2017 while
vessel was berthed at Kinder Morgan terminal, San Francisco. Unfortunately, the crew
succumbed to his injuries and did not show any vital sign as determined by paramedics
around 1930 LT. 8P was the last tank to discharge at this berth, carrying Coconut oil and
required squeezing operation as per charterer’s requirement.

Cause of incident still under investigation but reportedly due to slip and trip. However,
information received so far revealed that compliance with procedure was less than
adequate.

Trend in the industry and our company has been that compliance to the QSMS is a major
issue and single most common causal factor to the incidents. However, while a safety
campaign is in progress, a fatality reflects very badly on Company’s safety
management system and hard-earned reputation, including vetting approvals.

We hope you understand the seriousness of the issue and the need for full
compliance with the company procedure for safety operations.

As an immediate measure, kindly comply with the following until further notice

• Send your operation plan prior each loading, unloading and tank cleaning
operation to office for review by Marine PIC. Master to call Marine
Superintendent in suitable time prior to cargo tank entry for tank squeezing, tank
mopping, WWT etc to ensure that company entry procedure has been complied
with, entry permit was issued by responsible officer and verified by Master. No
tank entry shall be made without prior notification made to respective
designated Marine operations superintendent.

Additionally, kindly confirm


1) Receipt of recent DTN 02-2017 and usage of amended procedures and forms as
required.
2) Compliance with company emergency notification procedure without any delay
during an emergency.

Kindly immediately convene a SQC and disseminate and discuss this circular amongst
crew for safe compliance and acknowledge this circular at the earliest.
