1. PURPOSE
To ensures the safe and efficient operations
of cranes on site/installation.
2. DUTY AND RESPONSIBILITY
2.1 Construction Managers
Reviewing crane operations and in
particular identifying and marking those
areas where crane operations are restricted.
E.g. over “live” process plant, etc.
2.2 Appropriate Superintendent / Supervisors
 Ensure that all crane operators are
properly qualified and certified,
 Ensure that this Procedure and
manufacturer’s instructions are adhered
to,
 Ensure the correct tests and inspections
are performed
 Ensure that anchor patterns /
procedures have been established
before bringing any crane barge
alongside any project/site installation.
2.3 Safety Supervisor
Monitoring the safety of all working
practices involving crane operations
2.4 Crane Operators
 Ensuring the safe operation of the crane
at all times.
 Maintain an Official Log-Book in each
crane and log any mechanical, or
structural, defect and any accident or
unusual occurrence, all of which must be
reported to the appropriate
superintendent.
2.5 Banksman
In conjunction with the crane operator
ensuring that all lifting operations are
undertaken in a safe manner.

2. TUGAS DAN TANGGUNGJAWAB
2.1 Manajer Konstruksi
Memeriksa operasi crane khususnya
mengidentifikasi dan menandai lokasi
dimana pengoperasian cderek dilarang.
Mis., di atas pabrik proses yang “hidup” dll.
2.2 Pengawas /Superintenden Terkait
 Memastikan bahwa semua operator
derek memiliki kualifikasi dan sertifikasi.
 Memastikan prosedur ini dan instruksi
pabrik di patuhi.
 Memastikan bahwa semua pengujian
dan pemeriksaan dilaksanakan.
 Memastikan pola jangkar / prosedur
telah disediakan sebelum membawa
tongkang derek ke sisi proyek atau
instalasi lapangan.
2.3 Pengawas Keselamatan
Memonitor semua praktek keselamatan
yang melibatkan pengoperasian derek.
2.4 Operator Derek
 Memastikan keselamatan operasi derek
di setiap saat.
 Membuat buku catatan resmi pada
setiap derek dan catatan setiap cacat
mekanikal atau struktur dan kecelakaan
atau kejadian yang tidak biasa,
semuanya harus dilaporkan kepada
superintenden yang terkait.

3. DETAIL PROCEDURES
3.1 Crane Operators Criteria
Crane Operators must:
 Be over 18 years of age and have the
required MIGAS certification.
 Be adequately trained in the operation
of cranes, both the general principles
and the specific type of machine he is
required to operate.
 Be familiar with safe working practices
applicable to crane operation.
 Be medically fit with normal muscular
reflexes.
 Be able to read and understand
Operating Instructions and Procedures.
 Have a working knowledge of safe
slinging practices.
 Be familiar with and act upon the signals
used for the control of crane operations.
 Have sufficient working knowledge of
the crane to enable him to carry out the
routine maintenance checks for which
the operator is responsible.
Ensuring any new crane operator is allowed
working on a project site and given a
competence check by the relevant
Superintendent or Supervisor.
3.2 Pre Operating Checks
Ensuring a new crane is allowed to work on
any project site, ensuring its certification is
reviewed by the relevant Superintendent or
Supervisor.
At the beginning of each shift or prior to a
lift, the crane operator must carry out
following safety checks:
 Check that all lifting tackle is correctly
color-coded.
 Check hydraulic and coolant levels.
 Check that the audible alarms are
working for overload conditions.
3. RINCIAN PROSEDUR
3.1 Kriteria Operator Derek
Operator derek harus:
 Berusia > 18 th dan memiliki sertifikat
dari MIGAS.
 Terlatih secara baik dalam pengoperasin
derek, prinsip umumnya dan mesin jenis
tertentu yang harus ia operasikan.
 Mengetahui praktek kerja yang aman
sesuai dengan pengoperasian derek.
 Secara medis bugar dan memiliki refleks
otot yang normal.
 Dapat membaca dan memahami
Instruksi dan Prosedur operasi.
 Memilki pengalaman kerja praktek
slinging yang aman.
 Mengetahui dan bertindak terhadap
tanda-tanda yang digunakan untuk
mengendalikan operasi derek.
 Memilki pengalaman kerja dengan derek
yang cukup sehingga ia dapat melakukan
pemeriksaan pemeliharaan rutin yang
menjadi tanggung jawabnya.
Pastikan semua operator derek baru
diijinkan bekerja di lapangan proyek dan
diberikan pemeriksaan kompetensi oleh
superintenden atau pengawas yang
berwenang.
3.2 Pemeriksaan pra operasi
Pastikan semua derek baru diijinkan bekerja
di lokasi proyek, pastikan sertifikat telah di
periksa oleh superintenden atau pengawas
yang berwenang.
Pada awal shift atau sebelum shift, operator
derek harus melakukan pemeriksaan sbb:
 Periksa bahwa semua katrol pengangkat
diberi kode warna yang sesuai
 Periksa tingkat hidrolik dan pendingin

 Periksa alarm kelebihan beban
 Check that the load/radius indicator is
operating correctly. (No crane with a
capacity of over 1 ton should be allowed
to operate without an operational load
indicator).
 Check hydraulic actions, operating
motions and braking system.
 Check the crane cabin to ensure that no
materials that constitute a fire hazard
are present. Ensure a fire extinguisher is
present.
 Check radio communications (if the
operator and banksman are not in visual
contact throughout the complete lifting
operation).
 Check there is a lifejacket in the cabin
(for off shore cranes).
Before a lift is undertaken, the crane
operator must know the Safe Working Load
(SWL).
3.3 Operations
During crane operations check the
following:
 Unusual noises, loss of power or slow
response to control.
 All gauges for proper readings.
 The hoist drums to ensure that the wire
rope is spooling properly.
3.4 Wind Speed and Visibility
When wind speed exceeds 35 knots
extreme caution must be taken.
Only selected lifts, which can be lifted
safely, should be carried out. Types of lift
not recommended are items with a large
surface area, (e.g. steel plates), which could
be caught by the wind, giving rise to
hazardous working conditions.
When wind speed exceeds 40 knots,
operations may only proceed with the
permission of Construction Manager or a
responsible person delegated by him.
 Periksa bahwa indikator beban /

(Kapasitas Derek lebih dari 1 ton tidak
boleh beroperasi tanpa indikator beban).
 Periksa kekuatan hidrolik, gerak operasi
dan sistim rem.
 Periksa kabin derek untuk memastikan
bahwa tidak ada bahan-bahan mudah
terbakar. Pastikan ada pemadam api.
 Periksa radio komunikasi (bila operator
dan banksman tidak kelihatan untuk
melakukan operasi pengangkatan)
 Periksa ada jaket penyelamat di kabin
(untuk derek lepas pantai)
Sebelum pengangkatan, operator derek
harus mengetahui SWL.
3.3 Operasi
Selama operasi derek, periksa hal-hal
berikut:
 Suara bising yang tidak biasa, hilang
tenaga atau respon lambat dari sistim
pengendalian.
 Semua pengukur memberi angka yang
benar.
 Drum kerek untuk memastikan bahwa
tambang kawat tergulung rapi.
3.4 Kecepatan Angin dan Jarak Pandang
Bila kecepatan angin > 35 knots, maka
tindakan harus sangat hati-hati.
Hanya pengangkatan yang dipilih, mana
yang bisa diangkat dengan aman yang dapat
dilakukan. Jenis beban yang tidak dianjurkan
untuk diangkat adalah barang-barang
dengan permukaan lebar (mis., plat baja)
yang dapat diterpa angin, beresiko tinggi
yang membahayakan kondisi kerja.

Bila kecepatan angin > 40 knot Operasi
hanya diteruskan dengan ijin Manajer
Konstruksi atau orang lain yang mendapat
delegasi dari nya.
Prior to permission being given
consideration must be given to, but not
limited by, the following:
 Weather forecast
 Sea state
 Wind direction
 Type of lifting operation
 Visibility (especially during weathers of
darkness)
 Urgency of need with respect to plant
safety
The manufacturer’s instructions must
always be followed. In weather, which
causes inadequate visibility, crane
operations must be suspended.
3.5 Emergencies
Crane activities must cease in the event of
any emergency or potentially dangerous
situation. The crane operator must obey the
emergency stop signal who ever gives it.
3.6 Lifting Tackle
Latches or safety bolts hooks must always
be used. All loads must be checked for
correct slinging prior to lifting and all pipe
work should be pre-slung.
Only certified lifting tackle should be used.
The crane must not be used to lower, raise
or carry people unless authorized to do so
by the Construction Manager. The correct
equipment for carrying out such an
operation, i.e. a workbasket, must be used.
3.7 Suspended Loads
Loads must never be left suspended for
longer than is absolutely necessary. When
loads are left suspended, the crane controls
must never be left unattended. Personnel
should not be allowed beneath a suspended
load.
Sebelum pemberian ijin, hal-hal berikut
pantas dipertimbangkan tetapi tidak
terbatas pada:
 Ramalan Cuaca
 Keadaan laut
 Arah angin
 Jenis operasi pengangkatan
 Jarak pandang (khususnya pada cuaca
gelap)
 Kebutuhan yang mendesak dengan
memperhatikan keselamatan pabrik
Instruksi pabrik selalu harus diikuti. Saat
cuaca, dimana menyebabkan kurangnya
jarak pandang, maka operasi pengangkatan
harus ditunda.
3.5 Kedaruratan
Aktivitas derek harus dihentikan pada
keadaan darurat apapun atau situasi yang
berpotensi bahaya. Operator derek harus
mentaati kode stop darurat dari siapapun.
3.6 Katrol Pengangkat
Pelindung atau kait baut pengaman harus
selalu dipakai. Semua beban harus diperiksa
sling nya sebelum pengangkatan dan semua
pengangkatan pipa harus di memakai sling.
Hanya katrol bersertifikat yang dipakai.
Derek tidak boleh dipakai menurunkan,
menaikkan atau membawa orang kecuali
ada ijin dari Manajer Konstruksi. Alat yang
tepat untuk melakukan itu harus digunakan
mis. Keranjang.
3.7 Beban Tertunda
Beban tidak boleh di biarkan menggantung
lebih dari waktu yang diperlukan. Bila beban
digantung, maka pengendali derek tidak

boleh ditinggal. Personil dilarang berada di
bawah beban tergantung.
3.8 Crane and Load Movement
Loads being traversed on roofs and deck
areas must have sufficient clearance. Such
loads must not be slewed at any excessive
height. This is particularly important in
“Restricted” crane operating areas where
impact from “dropped objects” is critical
(i.e. wellhead areas).
Loads must be lifted gently and the crane
motions operated smoothly to avoid loads
swinging. Steadying lines must be used
where necessary or where the load presents
a large wind catching area.
When traversing a load on accommodation
or occupied offices / workshops, these
building must be vacated.
In general, lifting across “live” process plant
is forbidden. Permission should be sought
from the Construction Manager. Special
precautions may be necessary and
procedures produced.
If the rope becomes slack on the drum,
cross-coiled, or trapped, lifting operations
must be suspended until the rope is pad
out, examined for possible damage and re-
spooled correctly.
For all jib cranes with a capacity over 1 ton,
a safe load indicator should be fitted with
audible and visual alarm. The alarm starts
when the actual load being lifted is between
90% and 98% of the safe working load. In
addition there should be an audible alarm
outside the cabin when the actual load is
between 100% and 110% of the permitted
safe working load
Operating instructions/manuals and special
procedures must be available in the crane
cabin and supervisors office.
An audible alarm must be sounded prior to
slewing if personnel are at risk. Where
practicably possible, tag lines must be
attached to heavy, long or awkward loads.
3.8 Gerakan Derek dan Beban
Beban yang melintasi lokasi atap dan dek
harus cukup bebas. Demikian beban tidak
boleh diputar berlebihan pada ketinggian.
Ini sangat penting dalam daerah operasi
derek “terlarang” dimana dampak “barang
terjatuh” sangat kritis (lokasi kepala sumur).
Beban harus diangkat perlahan dan gerakan
derek harus halus untuk menghindari beban
berayun. Tali pengatur harus digunakan bila
perlu atau bila beban itu permukaan lebar
dapat tertiup angin.
Bila beban melintasi akomodasi atau kantor
/ bengkel yang dihuni, maka bangunan itu
harus dikosongkan.
Umumnya pengangkatan melintasi pabrik
proses yang hidup itu dilarang. Ijin harus
diminta dari Manajer Konstruksi. Penjagaan
khusus mungkin perlu dan prosedurnya
dibuat.
Bila tambang slip di drum, melilit, atau
terjebak, operasi pengangkatan harus
ditunda sampai tambangnya diluruskan,
diperiksa mungkin rusak, dan digulung ulang
dengan benar.
Semua jib derek dengan kapasitas lebih dari
1 ton, indikator beban dengan suara alarm
dan visual harus dipasang. Alarm berbunyi
bila beban yang diangkat berkisar 90 – 98%
dari beban aman. Sebagai tambahan harus
ada alarm suara di luar kabin bila berat
beban 100% - 110% dari beban aman yang
di ijinkan.

khusus harus tersedia dalam kabin derek
atau dikantor pengawas.
Alarm suara harus di bunyikan sebelum
berputar jika berisiko bagi personil. Bila
memungkinkan, tali pengatur harus
dipasang pada beban berat, panjang atau
berbentuk kaku.
3.9 Communications
The normal method of communication
between the banksman and the crane
operator should either be by hand signals or
two-way VHF radio.
All hand signals given to control crane
operations, must comply with the Standard
Code of Hand Signals. These must be clear,
distinct, easily seen by the crane operator.
Only the person in charge of the lift may
give signals to the crane operator (except in
an emergency).
3.10 Load Radius during Operation
There can be a noticeable effect on load
radius when picking-up or setting-down
loads due to stretch in jib suspension ropes
and jib deflection.
Loads must always be placed gently. When
operating near to minimum radius, the jib
must not strike the closed back steps when
the load is set down, otherwise the jib may
collapse or be damaged.
3.11 Safe Working Load and Maximum Working
Radius
The Safe Working Loads (SWL) of the crane
must not be exceeded except for the
purpose of a test, and then only by a
qualified test engineer, conforming to the
crane manufacturer’s specifications and
recommendations.
The maximum working radius of a crane
boom must never be exceeded with a load
on the crane hook.
When a load is equal, or nearly equal to the
Safe Working Load, the load should be
temporarily halted when just clear of the
deck to check the balance and security of
the load and the effectiveness of the brake.
3.9 Komunikasi
Metode komunikasi yang biasa digunakan
antara banksman dengan operator derek
adalah dengan tangan atau radio VHF
Semua kode tangan untuk mengendalikan
operator derek harus sesuai dengan Kode
tangan standar. Hal ini harus jelas, berbeda
dan mudah dilihat oleh operator derek.
Hanya orang yang bertanggungjawab pada
pengangkatan yang boleh memberi kode
tangan (kecuali pada keadaan darurat)
3.10 Radius Beban Ketika Operasi
Akan ada efek yang tampak pada radius
beban ketika mengangkat atau menurunkan
beban karena jangkauan tambang jib
suspensi dan defleksi jib.
Beban harus diletakkan perlahan, Bila
operasi mendekati radius minimum, jib
tidak boleh dekat mengenai sandaran
belakang ketika beban diletakkan, bila tidak,
jib mungkin runtuh atau rusak.
3.11 Beban Kerja Aman dan Radius Kerja
Maksimum
Derek tidak boleh melebihi Beban Kerja
Aman (SWL) kecuali untuk maksud
pengujian, dan kemudian hanya oleh
engineer uji yang berkualifikasi, sesuai
dengan spesifikasi dan rekomendasi dari
pabrik pembuat.

Radius kerja maksimum dari boom derek
jangan melebihi beban yang ada di kait
derek.
Bila beban sama atau hampir sama dengan
Beban Kerja Aman (SWL), beban harus
ditahan saat baru terangkat dari dek untuk
memeriksa keseimbangan dan keamanan
dan efektivitas rem.
3.12 Restriction on Crane Operations
Normal crane operations will be restricted
during the following:
 Helicopter operations
 When lifting over live process plant.
Permission must be obtained from the
Construction Manager. Extra safety
precautions may be necessary.
3.13 Shutdown
On completion of the work, the crane boom
and hook must be properly secured on the
boom rest and the motors stopped, before
the operator leaves the crane cabin.
3.14 Inspection Testing
 Ensuring each crane to be tested at least
one every 3 years and thoroughly
inspected within the last 12 months.
 After each boom wire change the crane
should be load tested.
 All ropes, chains and lifting gear is to be
inspected every 12 months. Gear to carry
personnel must be inspected every 6
months.
 A weekly inspection of the crane by the
crane operator especially of the safe load
indicator must be made.
4. RECORD
Inspection Testing record must be available
on the site or at office.

3.12 Batasan Pada Operasi Derek
Operasi normal derek akan dilarang apabila:
 Helicopter beroperasi
 Bila mengangkat di atas pabrik proses
yang hidup. Ijin harus diminta ke
Manajer Konstruksi. Mungkin perlu
pencegahan keselamatan tambahan.
3.13 Penghentian
Saat pekerjaan selesai, boom derek dan
kaitnya harus diamankan dengan benar di
sandaran boom dan mesin dimatikan
sebelum operator meninggalkan kabin
Derek.
3.14 Pengujian dan Pemeriksaan
 Memastikan bahwa setiap derek diuji
setiap 3 tahun dan secara menyeluruh
diperiksa dalam 12 bulan terakhir
 Setelah penggantian kawat boom, derek
harus diuji beban.
 Semua tambang, rantai dan mesin
pengangkat harus diperiksa setiap 12
bulan. Mesin pengangkat orang diperiksa
setiap 6 bulan
 Pemeriksaan derek mingguan oleh
operator crane khususnya indicator
beban aman harus dibuat
