REQUIRED DOCUMENTS FOR REPORTING FOR ADMISSION INTO MBBS COURSE

UNDER ‘C’ CATEGORY FOR THE ACADEMIC YEAR 2023-24

The following original certificates along with Xerox copies of 3 sets should be

submitted at the time of joining:

1. Online Registration Application

2. Receipt of Certificates acknowledged by convener TSMED-2021 Admissions for verification

3. NEET-2023 Hall Ticket

4. NEET-2023 Rank Card

5. Provisional Admission Order

6. SSC Marks Memo cum Pass Certificate

7. Intermediate Marks Memo cum Pass Certificate (Qualifying Exam Certificate)

8. Study Certificate (from VI to Intermediate)

9. Gap Certificate (If applicable)

10. Transfer Certificate

11. Caste Certificate of candidate and parent (for BC, SC & STs only) issued by Competent

Authority.

12. Minority Certificate issued by the concerned department. (for Muslim Minority)

13. Parental Income Certificate issued by MRO/Tahsildar.

14. Equivalency Certificate issued by Board of Intermediate Education, T.S., Hyderabad (for other

than Intermediate and studied out of Telangana State/Country).

15. Passport Size photos (10)

16. Aadhar Card (Xerox)

17. Course Discontinuation Bond for Rs.3,00,000/- with Notary.

18. Residence Certificate (Candidates who have studied in the institutions outside of Telangana have
to

submit 10 years (years of period to be specified) residence certificate of the candidate or either of
the

parent issued by MRO / Tahsildar excluding period of study/employment)

19. Proforma for Undertaking in the form of Affidavit (on Non-Judicial) Rs.100 /- Stamp paper by the

Parent and candidate stating that the caste and area mentioned in the certificates are genuine

and they will be held responsible for any further consequences as per law, if any discrepancy

noticed. (WITH NOTARY)

20. Candidates claiming eligibility of reservation under Special categories i.e., CAP, NCC, PMC and

Anglo Indian should furnish the required certificate in support of their claim.
