I/We hereby declare that the information provided in this application form is true and accurate. I/We have not withheld any details or
information, which might prejudice my/our application.

I/We hereby give my/our unconditional and unequivocal consent to the following:-

a. I/We authorise you or your representative to collect my/our personal information related to this application from my/our banker/other
financial institutions/credit reporting agencies/credit bureaus or from any source either directly or indirectly.

b. Pursuant to the Personal Data Protection Act 2010 and Credit Reporting Agencies Act 2010, I/We hereby expressly authorise you to
obtain from/ share, give, impart or disclose information regarding my/our personal data, credit information(s), credit report(s), credit
rating(s), credit score(s), financial standing related to this application with other financial institutions, Central Bureau of Bank Negara
Malaysia, Credit Bureau Malaysia Sdn Bhd (CBM), FIS Data Reference Sdn Bhd (FIS), CTOS Data Systems Sdn Bhd (CTOS),
Experian Information Services (Malaysia) Sdn Bhd (EXPERIAN), Central Credit Reference Information System (CCRIS), Dishonoured
Cheque Information System (DCHEQS), insurance company or agency or credit bureau or credit reporting agency or agency approved
by Bank Negara of Malaysia from time to time or Association of Finances Companies of Malaysia or Association of Hire Purchase
Companies Malaysia or to any other authority or body (government of otherwise) as you may in your discretion deem necessary or
required to under any applicable law, regulations, directives or guideline (having the force of law) and/or to any party whom in your
sole judgment considers is making enquiries with bona fide view to entering into a prospective transaction with you, or to any party
pursuant to any arrangement, composition, restructuring or any proposed arrangement, composition, restructuring with you as well as to
any collection agency or any other person appointed by you to assists you in collecting or recovering any sums due under this
Agreement for repossessing, recovering or disposing of the Goods/Vehicle;

c. I/We authorise you to disclose information about my/our credit information(s), credit report(s), credit worthiness, credit standing, credit
history and credit capacity with my/our proposed Guarantors or Indemnifiers (if any);

d. I/We fully aware and authorise you to use my/our credit information(s), credit report(s) and/or to obtain a further credit report or other
information (where necessary) from the above sources for the purposes inter alia as follows:-
i) to disclose or continue to disclose any credit information and/or to generate credit report(s) about me/us, my/our
company(ies)/business(es) to you at anytime;
ii) to assess, process or continue to process my/our credit information or my/our application in accordance to the provisions of the
Credit Reporting Agencies Act 2010;
iii) to conduct or continue to conduct credit checks on me/us for credit review, credit evaluation; debt recovery purposes and legal
documentation consequent to facility(ies) granted or continue to be granted by you to me/us;
iv) to notify or continue to notify a credit provider or credit reporting agencies or other financial institutions if I am/We are in
breach of any obligations you may have with me/us under any agreement to be entered with you in furtherance to this
application(s);
v) to exchange or continue to exchange credit information with other credit providers or credit reporting agencies as to the status
of my/our account where I/We are in breach of a contract with a credit provider;
vi) to assess or continue to assess my/our credit worthiness from time to time;
vii) to allow a credit reporting agency or credit bureau to create a file about me/us and to continue to update the information;
viii) to administer/monitor my/our account(s) from time to time;
ix) to monitor my/our performance, compliance or enforcement of terms under this application from time to time or any other
agreement entered with you in furtherance to this application;
x) for administration, marketing, planning and product development purposes; and
xi) any other purpose permitted or required by law.

e. I/We further agreed to waive all my/our rights and shall indemnify you and/or other financial institutions and/or credit reporting
agencies and/or credit bureau from any legal actions that may arise pursuant to the abovementioned consent.

f. I/We hereby confirm to have read understood, and agreed to be bound by your Privacy Statement (which is available at
https://www.toyotacapital.com.my/privacy-statement) as may relate to the processing of my/our personal information.

g. I/We solemnly declare that I/We has/have not in default under any agreement to which I am/We are a party or which I/We
may be bound and there is no pending litigation, arbitration, bankruptcy proceedings or winding up proceedings, as the case may be that
might materially affect my/our solvency. I/We further declare that I/We have not been convicted of any fraud, forgery,
misrepresentation, cheating, criminal misappropriation and criminal breach of trust or abetment of or attempt to commit such offence.
