CREDIT REFERENCE BUREAU CONSENT/ DECLARATION

1. Consent to Share Credit Information

I, the undersigned, hereby grant consent to Mushanga SACCO Ltd to share my credit
information with credit reference bureaus as permitted by law. This includes, but is
not limited to, information related to my credit history, outstanding loans, and any
other relevant financial information.
2. Purpose of Sharing Information
I understand that the purpose of sharing this information is to enable credit reference
bureaus to compile and maintain credit reports, which may be used by Mushanga
SACCO Ltd and other financial institutions when assessing my creditworthiness for
various financial products and services.
3. Duration of Consent
This consent shall remain valid for the duration of my relationship with Mushanga
SACCO Ltd and for a reasonable period thereafter as required by applicable laws and
regulations.
4. Withdrawal of Consent
I reserve the right to withdraw this consent at any time by providing written notice to
Mushanga SACCO Ltd. However, I understand that withdrawal of consent may affect
my eligibility for certain financial products and services.

bureaus will be treated with the utmost confidentiality and in compliance with
relevant data protection laws.
