their talents and aspirations, contributing significantly to its growth. Approximately 14%

2021). This article focuses on devising a care coordination strategy for the immigrant

community, with particular emphasis on the Mexican population, also known as Latinos
or Hispanics.

Justification for Addressing Healthcare Needs of Immigrants

Among immigrant populations in the United States, the Mexican community stands out
due to its size and specific healthcare challenges. Mexicans constitute one of the

largest immigrant groups in the country, accounting for 24% of the total immigrant
population (Ward & Batalova, 2023). Accessing healthcare facilities is often difficult for

Mexicans, leading to subpar healthcare experiences compared to other racial and


ethnic groups. Moreover, this community bears a disproportionately high burden of

chronic conditions such as diabetes, representing about 14.4% of the diabetes


prevalence within the community.

According to the Office of Minority Health (2021), Hispanics are 70% more likely to be
diagnosed with diabetes than non-Hispanics. Additionally, a significant portion of the
Mexican population lacks adequate medical insurance coverage, putting them at
increased risk of developing chronic health issues. Hence, there is an urgent need for
care coordination plans to address their healthcare needs. The selection process for
focusing on the Mexican population involved researching various immigrant groups in
the U.S., identifying the largest immigrant community in Virginia, and analyzing
healthcare challenges. These criteria led to the selection of the Mexican population.

Assessment of Healthcare Needs

A SWOT analysis was conducted to assess the healthcare needs of the Mexican

population. The community's strengths include adaptability and resilience, as evidenced
by their ability to navigate healthcare systems despite language barriers and
immigration status challenges. Moreover, the Mexican community fosters a strong

sense of solidarity, facilitating mutual support and resource access. However,
weaknesses such as healthcare disparities, language barriers, and higher rates of

chronic illnesses pose significant challenges to adequate healthcare access and
communication.

Social disadvantages, including education gaps, limited health insurance coverage, and
immigration status issues, contribute to healthcare disparities among Latinos (Oh et al.,

2020). Nonetheless, these challenges present opportunities for enhancing healthcare
delivery through culturally competent training, language support, and improved

insurance access. Deportation threats and hospitals' refusal to treat undocumented


individuals further exacerbate these challenges.

The National Alliance for Hispanic Health (NAHH) has partnered with various national
organizations to enhance healthcare access and practices for Hispanic communities.

Collaborations with medical associations and Spanish-language media aim to raise

awareness and provide bilingual health resources. Culturally competent training


initiatives and integrated healthcare services are being implemented to enhance
healthcare providers' practices and deliver personalized care to Hispanics (Office of

and community organizations play vital roles in promoting awareness and delivering

Characteristics of the Population

The Hispanic population, predominantly represented by Mexicans, constitutes a


significant portion of immigrants in the U.S. Demographically, a majority of Hispanics
primarily speak Spanish, highlighting language barriers to healthcare access.
Additionally, low educational attainment rates among Mexicans contribute to health
literacy gaps and limited access to healthcare services. Moreover, a substantial portion
of the Mexican population lacks health insurance coverage, with a significant number
being undocumented immigrants, further limiting healthcare access (Rosenbloom &
Batalova, 2022).

Current Healthcare Policies

Existing healthcare policies, such as the Affordable Care Act (ACA) and the National
Culturally and Linguistically Appropriate Services (CLAS), guide healthcare standards

to all Americans, undocumented immigrants remain ineligible for coverage. However,
policies like the CLAS prioritize cultural competence and language assistance to
