CORRECTIONS IN DEATH CERTIFICATE – CDMA

Commissioner and director of municipal administration, known as CDMA, registers and


maintains the Births and Deaths taken place all over A.P. Corrections in Death Certificate
service is useful when there is corrections required in existing record.

2. for Municipality INR 50/- Up To 4 Copies, INR 100/- for 5 to 10 Copies)
Documents Required ♦ 1) Application Form*

and Any one of the below documents

♦ 1)Original Death Certificate issued by the Registrar of


Death
♦ 2)In case of Medico Legal cases for death events,
certificate from the concerned police authority is a must
along with FIR and postmortem report
♦ 3)Notary Affidavit on Rs.10/- Non-Judicial
Judicial Stamped paper
♦ 4)Available Documentary evidences like educational
certificates,
ertificates, Election ID Card, Ration Card, Passport,
Driving License and Marriage certificates
♦ 5)A letter from the Hospital Authorities where the Death has
occurred

1) In Meeseva
seva home screen, under List of Services, select CDMA Department services as
depicted in Figure 1

2) In The list of CDMA Department services select Corrections in Death Certificate as depicted
in Figure 2

3) Now Corrections in Death Certificate Request screen will be displayed as

Note: - here we can process two types.
Type1: - Entering Registration Unit ID, Registration No and Registration Year.
Type2: - Clicking on “search” link.

4) Enter Registration Unit ID, Registration Year and Registration No which is taken from citizen
and clicking on “Get Details” button as depicted in Figure 4.

6) Now select Yes or No based on corrections required in Death request. Based on selection
fields will be displayed as depicted in Figure 6.

7) Enter details which are required Corrections as depicted in Figure 7.

8) Enter Informant such as informant name, Informant relation, Informant Address1, Address2,
Address3, Mobile No, Email ID, Remarks, Pin Code, Purpose Certificate and No.Of Copies as
depicted in Figure 8.

Note: - Here based on no. of Copies Service charges will be Calculated.

Note: Based on the Citizen request, the delivery option (either through Manual or Speed Post -
Local or Speed Post - Non Local at the Franchisee) has to be sselected.

10)) Collect the necessary supporting documents as indicated in the Corrections in Death
Certificate request service page scan the documents and upload into the system, as depicted in
Figure 10.

Note: - Here scan and upload all documents as single file

11) Click Show Payment. Another window appears requesting to Confirm Payment as depicted
in figure 11.

12) After Show Payment. Confirm Payment will be displayed with uploaded Documents and
payment details as depicted in figure 1

13)) Based on the delivery option chosen by the citizen, the service charges will be calculated
and displayed in Show Payment as depicted in figure 13.

14)) Collect the amount from the Citizen and click Confirm Payment to submit the request as
depicted in figure 14.

15) On confirmation, a receipt will be generated as depicted in the following figure 15. The
receipt will contain the Certificate delivery date.

Note: Ensure that you have sufficient balance with your service provider (SCA), else the system
will not accept the request

Note: - On submission of the request, CCitizen Receives Message. Like your Request for Corrections in
Death has been entered vide Application Number, Transaction Id and sent to Dept.

16) on submission, the request will go to respective Department workflow for further processing.

17) Once the request is approved by Department then, Death Certificate will be dispatched
through courier to the customer’s address if Delivery Type is Speed Post Local/Non Local.
Local

 If Delivery Type Is Manual, then the citizen need to collect the Death Certificate from
franchisee where he/she applied for the Certificate.
Type2: -

1) Operator clicks on “search” link as depicted in figure 1

2) Now, Birth Details Search window will be displayed as depicted in figure 2.

3) Enter Death details for searching record such as District, Location, Registration Unit ID,
ID
Child Name, Gender and click on get details button

4) After get details, Related requests are displayed as depicted in figure 4.

5) Select Related Request and Clicking on Submit button as depicted in figure 5.


6) After Submission, Registration Unit ID, Registration No and Registration Year are mapped to
main window as depicted in figure 6.

Note: - From this point onwards the process is the same as that of the “Type1”.
