Re: Medicine and Clinic Supply Request for April 2024 to June 2024
amounting to ₱ 4,931.50

Dear Father,

Good day!!

The school year is about to end our school clinic needs to be ready and equipped with
the essential medicines and other basic first aid kit paraphernalia needed for the SY 2023-2024
end also covering the Summer Break. The medicines are usually needed to answer simple
health concerns that require medicinal interventions or other emergent situations for the
employees, our Teachers/Staff and of our students. We especially need these for our current
situation during this increased health risks brought by the changing times and weathers. We
need to be at our best and optimum health and to be free from sickness in performing our
duties and work at our finest state. We would like to request for the replenish of medicines that
are now out of stock due to continuous usage/consumption during the 3-4 months period of
the previous quarter.

May I humbly request for the purchase of medicine and other clinic supply with the total
amount of ₱ 4,931.50

The said medicines are estimated for 3-4 months’ consumption for the population of our
student this school year 2023-2024. Please find the attached breakdown of medicines needed
for purchase and their corresponding amount for your reference.

Should the said request be approved, immediate liquidation shall be submitted

supported by original receipts.
