 Do NOT perform this practice until you have been appropriately trained and authorized to do so by your supervisor.

General Safety Requirements and Instructions


Do
 Recognize a confined space as one that is enclosed or partially enclosed, and is not designed for continuous human occupancy,
yet is large enough and configured such that you could enter to perform work, and has limited or restricted means for entry or
exit.
 Review applicable written safe work procedures prior to entry into a confined space.
 Ensure pre-entry testing and inspection of the confined space has been conducted, based on the written procedures
 Follow precautions identified in the written safe work procedures, and follow any other precautions necessary for the health and
safety of workers.
 Know the hazards which may exist prior to entry or may develop during work activity inside the space.
 Know the means of controlling hazards which are expected for the space prior to entry, or which may develop during work
activity inside the space.
 Be prepared and know the steps to take if any unexpected situations arise during work activity inside the space.
 Obtain all personal protective equipment specified and designed for entry into each confined space.
 Affirm that any lock-out has been performed AND documented prior to entering. Ensure that any adjacent piping has been
blocked, bled, blanked, blinded or otherwise disconnected.
 Affirm that the atmosphere has been tested and documented with approved gas monitoring devices prior to entering.
 Know the location of the posted confined space entry permit, when applicable.
 Affirm that standby persons are continually monitoring the atmosphere, and your well-being, and are prepared for rescue or
extraction from the space, as applicable.

Don’t
 Enter a designated, or even suspected confined space, unless trained and authorized to do so. Confined spaces can be deadly.
 Enter a confined space unless all hazards are eliminated or minimized and work can be performed in a safe manner.
 Enter a confined space unless your employer has prepared and implemented a written confined space entry program that

includes a hazard assessment of that space along with written safe work procedures for entry into and work in that confined
space.
 Enter a confined space unless the responsible supervisor or person assigned responsible has performed pre-entry testing and
inspection of the confined space based on written procedures.
 Partially enter a confined space unless you are authorized.
