We, the Grade 10 students of Section Charity from Kapayapaan Integrated School,
are writing to seek your permission to conduct a survey as part of our research study on
the “Mental Health Services and Grade 10 Students Well-Being”.

We are kindly asking for your permission to conduct a survey among the grade 10 students in
Kapayapaan Integrated School. The survey will be through an online form specifically Google
Forms. The survey will only take a little amount of time between 3-5 minutes to be finished.
The purpose of this survey is to gain an insight about the mental health well-being of my fellow
grade 10 students and what is the efficacy of the mental health services of Kapayapaan
Integrated School. We assure you that this survey is not compulsory and will be purely
voluntary. The information gathered will be kept private/confidential throughout the study and will
only be used for academic purposes.

We will humbly acknowledge you for allowing us to conduct this survey and giving us permission
to distribute the questionnaires to grade 10 students of Kapayapaan Integrated School.
We thank you for your patience and time reading this survey letter and we are asking you for
your consideration and approval of conducting this survey.
