Project: Ministry of Higher Education Promo Video

I hereby agree to the following:

• To allow Taylor’s University or its representative to use my name, likeness,
image, voice, and/or appearance as such may be embodied in any pictures,
photos, video recordings, audiotapes, digital images, and the like, captured of
me.
• Taylor’s University have complete ownership of such pictures, etc., including
the entire copyright, and may use them for any purpose consistent with the
Taylor’s University missions.
• The materials may be used as aforesaid by the institution to represent an
imaginary person.
• The materials may be used to illustrate any statement and/or wording which
the institution may desire or think fit to use in any medium now known or
later developed, including the Internet.
• Any images retouched, altered or worked-up and all productions thereof shall
be deemed to represent an imaginary person and would not implicate me in
any way.
• Lastly, I acknowledge that I will not receive any compensation, etc. for the use
of such pictures, etc., and hereby release the Taylor’s University and its
agents and assigns from any and all claims which arise out of or are in any
way connected with such use.

I have read and understood this consent and therefore:

I give my consent to the Taylor’s University to use my name and likeness to
promote the Taylor’s University program and/or their activities.
