AFFIDAVIT OF SELF ADJUDICATION

I, LYZIEL FABALIÑA TANATE Filipino, of legal age and residing Block
15, Lot 3, Emerald St., Ciudad Grande Subdivision, Brgy. Muzon, Taytay,
Rizal after having been duly sworn to in accordance with law, hereby depose
and state, that:

1. That I am the biological niece of GLECY FABELLA CORTAS, who

died on April 29, 2023 in Pasig, City, as evidenced a copy of her
Certificate of Death is hereto attached as Annex “A” hereof;

2. That GLECY FABELLA CORTAS, is the biological mother of JOHN

RACY ADAP a.k.a. “JOHN RACE HADAP” @ “RAIN”, who died on
September 8, 2022 in Muntinlupa City, Metro Manila, as evidenced
a copy of his Certificate of Death is hereto attached as Exhibit “B”
hereof;

3. That at the time of death of JOHN RACY ADAP a.k.a. “JOHN RACE
HADAP” @ “RAIN”, he posted bail for his provisional liberty in the
above-entitled case as evidenced by Official Receipt (OR) No.
2695518 dated July 9, 2014, as evidenced a copy of said Official
Receipt is hereto attached as Exhibit “C”, hereof, which bail needs
to be release as per order of the OCC;

4. That, pursuant to Rule 74, Sec. 1 of the Rules of Court, I hereby

adjudicate unto myself the above-mentioned bail by means of this
affidavit and hereby files the same with the RTC Manila Branch 28,
with the request that the said adjudication be made effective
without judicial proceeding as prescribed by the aforementioned
provision of the Rules of Court.
