1. Latar Belakang
Proyek senipah Balikpapan telah memasuki masa precommissioning & - INFO
commissioning dimana membutuhkan koordinasi dengan pihak KPI, KPB,
PHM, PHKT, dan RU V.
2 Umum :
a) Pembukaan oleh bapak Kusuma Darmono selaku PM Senipah –
Balikpapan – PT Pertamina Gas menyampaikan progress pekerjaan Pipa - INFO
Gas Senipah – Balikpapan sebesar 85.78% per tanggal 25 Juni 2023.
b) Pada Hari Pertama Meeting Persiapan Commissiong Gas In Pipa Gas
Senipah - Balikpapan, PTG telah menyampaikan :
- Operating Philosophy, Organization Chart, Hasil Flow Assurance (pada
Case 1, Case 4 dan Case 6), Metering System, Gas Loading - INFO
Calculation dari Fasilitas Metering Senipah – Balikpapan sesuai
dengan Slide Presentasi pada Lampiran 1.
- Precommissioning, Commissioning & Simop sesuai dengan Lampiran - INFO

c) MOM ini akan menjadi dasar untuk masing-masing entitas dapat membuat
surat tugas team-nya (jika diperlukan) dalam rangka men-support - INFO
commissioning Pipa Gas Senipah - Balikpapan.
d) Pada Hari Kedua Meeting Persiapan Precommissioning & Commissioning
Gas In Pipa Gas Senipah - Balikpapan, PTG telah menyampaikan : - INFO
- Lanjutan pembahasan Precommissioning & Commissioning, Operating
and Maintenance Strategy, lanjutan pembahasan Gas Loading
Calculation (Lampiran 3), lanjutan pembahasan Draft Gas Delivery
Procedure.
e) PTG menginformasikan Meter di Balikpapan memiliki sertifikasi dan - INFO
spesifikasi yang sama dengan Meter Senipah sebagai titik serah.
f) Telah dilakukan pembahasan draft Gas Delivery Procedure, Gas Loading PTG OKAL W2 Juli
Procedure, akan dilanjutkan pada meeting selanjutnya dengan pihak 2023
terkait.
g) Operating & Maintenance (O&M) Agreement antara PTG dan PHI Group PTG OKAL W2 Juli
akan di-update oleh PTG dan dikirimkan kepada PHI Group untuk di- 2023
review.
h) Terkait operasi dan pemeliharaan di sisi hilir mengacu kepada Perjanjian - INFO
Kerjasama Pembangunan dan Pemanfaatan Fasilitas Penyaluran Gas
Bumi RU V Balikpapan antara PTG dan KPB.
3. Pembahasan :
a) Responsibility dari entitas (KPI, KPB, RU V, PHKT, PHM) sesuai - INFO
dengan Lampiran 4.
b) Dari KPI, KPB, RU V, PHM, PHKT telah memberikan nama-nama PIC - INFO
untuk dimasukkan ke team precommissioning dan commissioning
sesuai dengan yang tertera pada Lampiran 4.

INTRODUCTION

A Health, Safety, and Environmental (HSE) Management Plan should be developed


for the project. In addition, other plans and procedures to cover the inherent risks
associated with pre-commissioning, commissioning, and other activities as well as
risks associated with the performance of these activities while others are working in
adjacent areas of the facility.
These plans / procedures may consist of, but are not limited to:
1. Identification of systems under the control of commissioning
2. Pre Start-up Safety Reviews (PSSRs)
3. Permit to Work system
4. Hazard identification and risk management
5. Simultaneous Operations (SIMOPS)
Of particular concern are facilities with multiple train / process units where
construction and commissioning activities may be occurring in one area of the
facility simultaneously with start-up and operations (by the Client) in another area

DEFINITION

A SIMOPS (simultaneous operations) is carried out in order to identify the risks


associated with the various simultaneous operations of a project or process during
the hazard identification study based on the project execution sequence. The
review identifies possible interactions between activities that may adversely impact
people, property, or the environment. SIMOPS reviews are an important addition
to the performance of Hazard Effect Management Process Studies (HEMP) or PHA
studies such as HAZID, Hazard and Operability (HAZOP) studies etc.
SIMOPS typically involves multiple stakeholders (owners, contractors, sub-
contractors and vendors), large multi-disciplined workforce’s and a range of daily,
24-hr, routine and non-routine construction, commissioning and operation
activities. Simultaneous operations activities can significantly increase the risk

Before performing SIMOPS, a site-specific SIMOPS operating plan should be
prepared. This plan is the outcome of the SIMOPS assessment review carried out
before any activity begins. The SIMOPS assessment review, which is also based
on the scope of work (SOW), will capture all the phases/activities planned and the
correlated hazards (The HAZID - Hazard Identification is part of the SIMOPS
assessment review).

The SIMOPS risk assessment process must review and evaluate in detail the
simultaneous operations and tasks, including the following steps:
 summarize the operations that may take place simultaneously in the field and
identify the hazards capable of arising because of these;
 investigate the development of the hazards into accident events;
 assess the effectiveness of hazard controls in such circumstances; and
 identify the consequences of accident events, where they have been shown to be
able to occur, to determine the effects on the facility risk levels.
SIMOPS shall be identified by responsible personnel at all stages of PTW lifecycle.

PRE-COMMISSIONING

Pre-commissioning is primarily done to ensure that all pipeline and metering


station facilities construction was satisfactory and meets the standard safety
specifications. Commissioning activities can only be carried out after all
system/sub-system have been duly pre-commissioned and determined to be
safe for initial testing.

To avoid accidents and ensure the safety of the manpower involved in all aspects
of pipeline pre-commissioning and commissioning, the following steps must be
taken :
 Proper training and briefing of all personnel on their safe performance of their
duties, Communication system, knowledge of emergency shut down and
evacuation plans with the designation of multiple points of egress
 Diligent instruments/apparatus (electrical wiring inspection, lubricant checks)
with the replacement of faulty parts
 Provision of and mandatory use of personal protective equipment

COMMISIONING

The supervision of the commissioning execution activities is done by Subcontractor


and the manpower and tools are provided by CONTRACTOR.
The commissioning phase essentially includes following tasks:
 Preliminary checks (Verifications for Ready for commissioning status)
 Functional tests (Dynamic functional tests, energized)
 Operational tests (On line tests of utilities and major equipments)
 Piping and Vessels preparation (Activities related to piping dry-out, process leak
testing, inverting).
Pre-commissioning and Commissioning activities will take place simultaneously with
Construction activities and as such will require clear controls to avoid loss, impact or
delay. This will be achieved through defined permit to work, LOTO processes, risk
management, planning and establishing SIMOPS register. All works will be
supervised by competent personnel who have a good understanding of the works,
its impacts and the HSE controls necessary to prevent loss.

Prevention of Accidental Ignition


• Source of ignition should be removed and adequate fire extinguisher
should be provided
• Provide signs, flagmen or guard to warn about the hazardous area
• Accidental ignition by electric arcing can be prevented by connecting
the cables to each side of any piping that is to be parted or joined
• Welding process and cutting process using torch should be done by
checking the combustible gas mixture

Taken when unusual operating and maintenance conditions occur, such as
failures and leakage. If not satisfying, a planned program shall be initiated to
abandon, replace or recondition and proof test
 Pipeline Patrolling
to observe surface conditions on and adjacent to each pipeline right-of way,
indications of leaks, construction activity and any other factor affecting the
safety and operation

Types of surveys shall be effective for determining if potentially hazardous
