1. INTRODUCTION
The purpose of this procedure is to control any hazards associated with confined space entry to as low as
reasonably practicable, and ensure all personnel have the minimum guidelines to execute the work in the
confined space safely.

2. SCOPE
This procedure applies to all personnel, Subcontractors and vendors. This procedure will apply to all confined
spaces encountered during the Tank Bottom and Roof Coating and External Painting including, but not limited
to:
• Large diameter pipelines;
• Tanks, space above floating roofs and floating roof tanks;
• Process vessels;
• Ducts and flues;
• Sewers, sumps, drains and culverts;
• Manholes, pits, trenches and excavations deeper than 1.2 meters, unless a confined space risk
assessment is carried out to determine otherwise (refer to Safe Excavation & Trenching Procedure,
DRC-000-16540-0044);
• Chambers, etc.

4. DEFINITIONS & ABBREVIATIONS


4.1. Definitions
The following definitions shall be applied for this project.

Contract The agreement entered between (ExxonMobil Iraq Limited) as a contractor Dr. Concrete.
Authorized Entrant A person who has received adequate training to enter and work in a confined space.
Blinding The absolute closure of a pipeline, or duct by the fastening of a solid plate that completely
covers the bore and that can withstand the maximum pressure of the pipeline or duct with
no leakage beyond the plate.
Competent Person A person designated by management who has the practical skills, experience and
knowledge to undertake a task to a recognized, safe standard considering all predictable
hazards associated with the work and the environment or surroundings in which it is
undertaken.
Confined Space An enclosed or partially enclosed space that:
• Is not primarily designed or intended for human occupancy.
• Has a restricted entrance or exit by way of location, size or means.
• Can represent a risk to the health and safety of anyone who enters, due to
one or more of the following factors:
- its design, construction, location or atmosphere
- the materials or substances in it
- work activities being carried out in it, or the
- mechanical, process and safety hazards present
Confined Space A trained and qualified individual stationed outside a confined space with the responsibility
Attendant of ensuring the safety of authorized entrants through the performance of pre-entry
briefings, checking and inspecting PPE, equipment and tools and performing regular
confined space monitoring in ensuring space remains safe for entry and occupation.
Confined Space Entry The action by which a person passes through an opening into a confined space. Entry
includes ensuing work activities, inspection and survey and under the control of a Permit
to Work.
Emergency Any occurrence (including any failure of hazard control or monitoring equipment) or event
internal or external to the project that could endanger employees.
Entry Register A dedicated sign in/out logbook giving details of timings of all personnel entering and
exiting the confined space.
Hazardous An atmosphere that may expose employees to the risk of death, incapacitation,
Atmosphere impairment of ability to self-rescue (that is capable of escape unaided from a confined
space), injury or acute illness.
Hot Works Operations such as welding, grinding or gas cutting, which are a source of ignition.
Occupational Exposure The maximum concentration of an airborne substance, averaged over a reference period
Limit to which employees may be exposed by inhalation under any circumstances.
Oxygen Deficient An atmosphere containing less than 19.5 percent of oxygen by volume.

Oxygen Enriched An atmosphere containing more than 23.5 percent of oxygen by volume.

Rescue Plan A written plan of actions that has provisions for conducting a timely rescue of individuals
within a confined space, should an emergency arise.

5. RESPONSIBILITIES
5.1. PTW Issuing Authority
 The person designated as Issuing Authority shall undergo the relevant training and must pass the
evaluation assessment.
 The issuing Authority is responsible for ensuring that the controls specified on the Permit to work (PTW)
are applied;
 Identification of hazards and controls to allow work to be conducted safely and to ensure that isolations
are applied;
 Ensure that there is no conflict with the work of other groups within his area of responsibility or in
adjacent areas;
 Communication to all parties that need to know of the work;
 Ensure all applicable forms and certificates are attached to the PTW prior to issue;
 Ensuring that the Performing Party fully understands the conditions of the permits;
 Authorizing and issuing the PTW;
 In conjunction with the Performing Party, authorize who may enter the confined space;
 At shift change, the outgoing Issuing Authority must inform the incoming Issuing Authority of the status
of all PTW’s within his area of responsibility;
 Regularly consult with the Performing Party for status of outstanding PTW’s not currently in use,
cancelling those no longer required;
 Ensuring a second check of all isolations is made prior to initial issue of a PTW;
 Closing PTW’s, e.g., as soon as the Performing Party has submitted and signed off that the work has
been completed and the area re-instated;

5.2. PTW Performing Party


 The person designated as the Performing Party shall attend the relevant training and must pass the
evaluation assessment.

 Investigate the need for the confined space entry, i.e. can the task be completed without personnel
entering the confined space;
 Ensuring tested rescue plans are in place and equipment available;
 Ensuring hazards are identified and controlled for the work being undertaken in the confined space, as
well as the hazards of the confined space;
 Ensuring a JSA has been carried out and controls are in place;
 Maintain a register of personnel who are authorized to enter the confined space. A copy of the register
shall be given to the Issuing Authority;
 Ensure that hazards and associated risks are identified and communicated to the relevant workforce;
 Ensure that responsibilities with respect to confined space entry are safely and effectively transferred;
 Authorize confined space entry and allow entry to begin (provided that the requirements of this
procedure have been met);
 Ensure that effective and well-maintained gas monitoring is provided, and that rescue services and
equipment are available and that the means for summoning those services are operable;
 Ensure that appropriate personal protective equipment (PPE) is issued to, and worn by, personnel
entering a confined space;
 Ensure acceptable confined space entry conditions are maintained and that they remain consistent with
the terms and conditions specified in the Confined Space PTW;
 Ensuring that personnel are competent and physically fit for their specific role, including training for
personnel entering the confined space in hazards thereof;
 Conduct a STARRT meeting with all personnel involved in that entry including the Area supervisor, Area
Operator, work group personnel and Rescue Team members etc. This meeting will cover the scope of
work, rescue plan, job responsibilities and safety issues of the entry;
 Ensuring that the work/rest periods are established;
 Organizing, allocating and briefing the Confined Space Attendant;
 In conjunction with the PTW Issuing Authority, authorize who may enter the confined space;
 Remain near the work area or be readily accessible to and in regular contact with the team working
under the permit;
 On the sounding of an emergency alarm, understand that the permit is automatically suspended and that
the work team must make the work site safe and leave the work area.

5.3. Authorized Gas Tester


Only personnel who have attended and received the certificate as Authorized Gas Tester (AGT) from an
approved third-party training center are considered AGT.
They shall perform the following tasks:
 Ensure the gas detection equipment is third-party certified, calibrated and conduct necessary set up
tests on the equipment as per the manufacturer’s manual instructions.
 Record on the Gas Test Certificate the times and results of the initial gas test and any requested follow-
up gas tests.
 Take immediate steps to halt work if gas tests results are outside the required range and bring it to the
attention of the responsible Supervisor.
5.4. Contractor/EPC Subcontractor and Subcontractor Field Supervisors
Only personnel who have attended the Confined Space Attendant Training and successfully passed the
evaluation assessment and obtained a valid identification card are considered Confined Space Attendants.

All Confined Space Attendants are required to follow these minimum requirements:
 Maintain an accurate count of all persons inside the confined space by:
- Using a tally board and logbook on which the name, entry and exit times for all personnel
entering or leaving the confined space shall be recorded; and
- Ensure that air lines and/or safety lines are marked so that everyone inside a confined space
is clearly identified in the event of a problem;
 Be aware of the hazards that may be faced during entry, including the mode, signs or symptoms, and
consequences of any exposure;
 Monitor conditions and activities inside and outside the confined space to determine if it is safe for
entrants;
 Ensure effective communication systems for the entry are tested;
 Maintain effective and continuous communication with authorized entrants during entry;
 Order authorized entrants to evacuate the confined space immediately if:
- A condition is observed that is not allowed;
- Behavioural effects of hazard exposure are detected;
- A situation occurs outside the confined space that could endanger the health and safety of
authorized entrants;
- An uncontrolled hazard is detected inside the confined space; or
- The attendant must leave his workstation.
 Summon rescue and other emergency services in the event of emergency situations;
 Take necessary actions when unauthorized persons approach or enter a confined space without proper
authorization or when entry is already underway;
 Recognize and respond to abnormal conditions inside and outside the confined space;
 Ensure gas test requirements as defined in the permit and attached certificates are met;
 Ensure specified rescue and back-up equipment are available at the entry point;
 Signal to those inside the confined space to vacate the area in the event of emergency alarm sounding;
 Verify the status of those working in the confined space on a regular basis;
 Ensure that all the safety requirements and precautions are effectively in place before entry and for the
duration of entry;
 Shall not, under any circumstances, enter the confined space;
 While the confined space is occupied, the Confined Space Attendant shall have no other duties;
 Shall not leave his post unless relieved by another fully trained Confined Space Attendant and a
complete hand-over of conditions, personnel involved etc. has been carried out;
 Ensure that the work/rest periods are observed;
 Shall ensure all permit copies and attachment certificates are posted on the Tally Board;
 Where equipment such as ventilation equipment, fall protection, retrieval apparatus, atmospheric
monitoring equipment, SCBA has to be monitored to ensure the safety or personnel in a confined space,
the Confined Space Attendant can only be used for these additional monitoring roles if they do not
impact on his ability to perform the prime safety watch duties. Where this is the case, additional
personnel should be assigned for the purpose.

5.5. Authorized Entrants
Only personnel who have attended the Confined Space Entry Training, successfully passed the evaluation
assessment and obtained a valid identification card are considered Authorized Entrants. All authorized
entrants are required to follow these minimum requirements:
 Adhere to established safety standards and practices;
 Knowing and recognizing the hazards that may be faced during confined space entry, including the signs
or symptoms and consequences of exposure to hazards;
 Making proper use of equipment and protective devices;
 Maintaining communication with the Confined Space Attendant to enable the attendant to monitor the
entrant's status. They must also alert the entrant of the need to exit the confined space if any hazards
are identified;
 Alerting the Confined Space Attendant if a prohibited condition exists or when symptoms of exposure to
confined space hazards are observed;
 Exiting from the confined space as soon as possible when:
- Ordered by the attendant;
- The entrant recognizes the warning signs or symptoms of exposure; or

- A prohibited condition exists.

6. PROCEDURE
6.1. General
Confined spaces can possess many hazards that are required to be assessed prior to authorized entry.
activities shall be planned in a way where Confined Space Entry is only authorized if no other method is
available to perform the work.
Confined spaces shall be made safe for entry using one or more combined methods to ensure all hazards
have been removed or controlled prior to entry by following the minimum guidelines described in this section.
The actual preparation of a confined space will vary according to its design, construction and use. The
following process shall be assessed prior to authorizing access to any confined space:
6.2. Communication
All possible confined spaces during Tank Bottom and Roof Coating and External Painting activities shall be
identified during the constructability studies or any other available method in the project. Those areas
identified as confined spaces shall be assessed and access shall be controlled.
6.3. Risk Assessment
Prior to the performance of any work in a confined space, a SWMS and a Specific JSA must be completed,
recorded in writing and approved.
The following minimum health and safety concerns shall be considered:
 Oxygen deficiency
 Oxygen excess
 Operation of moving equipment
 Presence of contaminants (toxic/harmful gasses/atmosphere)
 Ionizing radiation if any
 Introduction of steam, water, gas or liquid
 Electrocution
 Fire and explosion

 Difficulty of access and egress
The number of personnel for entry is related to the specific work to be performed. This will be decided during
the JSA session. Considering the results of the above mentioned JSA, the confined space will then be
prepared and placed in a safe condition for entry (prior to issuance of the PTW).
Engineering controls such as cleaning, flushing, isolation, bleeding, LOTO etc. shall be identified at this stage
and incorporated into the JSA.
6.4. Permit to Work
Entering and performing operations or activities in a confined space shall be controlled in accordance with the
Permit to Work Standard. Every entry into a confined space for inspection or work shall require the intended
entrants to apply for a PTW. The confined space entry will only be allowed for the period stated in the
approved PTW.
6.5. Entry Controls
A person designated as Confined Space Attendant shall be posted at the access opening with an entry board
for access control before anyone enters the confined space.
The Confined Space Attendant shall be trained and shall be specifically named on the Permit to Work. Where
an entry is ongoing to the extent that the Confined Space Attendant will change from time to time, the name of
the company or the discipline (e.g. Civil) will be noted on the permit.
The Confined Space Attendant will record the name, time in and out in a Confined Space Entry Log Sheet,
such as the sample log sheet in ATTACHMENT-2, to ensure that:
 During the time that an entry permit is in operation it is known when any person is in the confined space.
 All personnel have left the space on completion or suspension of the work.
 There is a record in writing of the above.
 The Performing Party is in overall control of the entry and task. The Confined Space Attendant cannot
authorize someone to enter a confined space.
 If a confined space is open but entry has not been authorized or is not in progress, then entry points
must be barricaded, e.g. 'Danger - Confined Space - No Entry' sign or danger tape must be secured
across the entry.
 The entry way to the confined space must be kept clear of any obstacles at all times.
 A person, whose head or upper body is within a confined space, is considered to have entered that
confined space.
 Each person entering a confined space shall place his Site ID in the slot provided on the entry board.
 When electric tools are to be used, a Residual Current Device (RCD) must be used to protect them.
 Plant air must not be used for respiratory supply or for replacing vented vessel atmospheric air when
people are inside.
 All portable lighting must be supplied through a residual current device. It is preferable to have 12-volt
explosion proof/intrinsically safe lighting.
 High heat output lamps must not be used.
 Equipment not being used for the task at hand shall not be stored within a confined space.
 Any fixed fire extinguishing equipment inside the confined space should be positively isolated when the
space is occupied, regardless of whether it has manual or automatic activation controls.
 When arc welding works are suspended for a substantial period, such as during lunch or overnight, the
power source to the equipment shall be de-energized, all electrodes removed from holders and holders
placed so that accidental arcing or contact cannot occur.

 When gas welding or cutting equipment is not in use the torch and cylinder valves shall be closed, the
torch and hose removed from the confined space and depressurized outside away from the confined
space.
 No compressed gas cylinder or associated manifold, other than those used for self-contained breathing
apparatus, shall be located inside the confined space (Air-line BA is preferable).
 Continuous atmospheric monitoring shall be conducted whenever the confined space is occupied.
 The work/rest periods routine must be established prior to entry.
 The work/rest periods must also be established for the Confined Space Attendant and Rescue
personnel.
 Personnel shall also carry handheld flashlights of an appropriate hazardous area rating with fresh
batteries for use in the event of loss of temporary lighting. In very small spaces, e.g.
 inside skirts of vessel, the use of handheld flashlights would be acceptable alone provided they were of
an appropriate hazardous area rating.
6.6. Pre-Entry Briefing and Checks
Before entry is permitted, the Performing Party shall brief entrants and attendants on their responsibilities, the
hazards within and controls for safe entry.
Pre-entry briefings shall be conducted outside a confined space in the form of a Toolbox
Talk/STARRT meeting. Such briefings shall include information such as, but not limited to, the following:
 Potential hazards and its associated risks.
 Signs and symptoms of potential hazards that could arise.
 The prevention, control and mitigation measures preventing the occurrence of an incident.
 Communication, emergency and rescue procedures.
 Checking and use of PPE.
 Reporting of illness and incident.
During this pre-entry briefing, the Performing Party or Confined Space Attendant will use a checklist, such as
the sample checklist enclosed in ATTACHMENT-1, to confirm that all necessary preparations have been
carried out to ensure safe entry into the confined space. They shall also inform the rescue team and Medical
Centre of the entry.
6.7. Communication
An effective, dependable communication system between the CSA and the entrant(s) shall be maintained
always when the entrant(s) are in the confined space.
If the attendant cannot directly and effectively communicate visually or by voice with the entrants, effective
means of communication shall be made available (e.g. IS Rated Radios, 2-Way Radios and other similar
devices). IS devices shall be the preferred communication device in IDLH atmospheres and if communications
are interrupted, entrants shall be evacuated.
Hazard communication shall be posted at the entry point of the confined space. Other necessary information
such as emergency contact numbers, person in charge, etc. shall be posted and communicated to all workers
during the pre-entry briefing.
6.8. Atmospheric Monitoring
Contractor/EPC Subcontractor and Subcontractors shall provide fully functional and regularly calibrated Multi-
Gas Monitors.
An atmospheric test shall be conducted at the beginning of each shift prior to the entry to the confined space,
and at different intervals if required in the PTW.

Personal gas detectors may sometimes be used inside confined spaces by entrants, including deep
excavations, as a precautionary measure. Any gas test required on the PTW shall be conducted and recorded
by an AGT.
6.9. Ventilation
Whenever possible the confined space shall be ventilated to enable entry without respiratory protection.
Ventilation shall ensure the circulation of air within the confined space without stagnant pockets. Ventilation
can be used to reduce or remove the atmospheric hazard created while personnel are inside the confined
space. Mechanical ventilation is preferred over natural ventilation.
Air intake of forced ventilation equipment shall be from an uncontaminated location. When possible, two or
more manways shall be open in every chamber of space for proper ventilation. Where it is not practicable to
ensure a safe oxygen level or atmospheric contaminants cannot be reduced to below the relevant exposure
level, entry to the confined space will only be permitted with suitable PPE including supplied air respiratory
equipment.
All entrants shall wear personal H2S monitors where/when H2S might be present.
6.10. Rescue Plan
Specific rescue plans shall be developed and include as a minimum:
 An assessment on the hazards associated with the confined space;
 Required gas testing / monitoring equipment;
 Personnel required and trained to perform the rescue;
 All precautions to be taken while in the confined space;
 Required PPE;
 Required rescue equipment;
 Required tools and any other specialist rescue equipment.
A means of communication shall be provided, and a system of signals agreed and understood by all
personnel involved. These communications arrangements shall be maintained throughout the duration of the
entry.
In all cases of confined space entry, the trained attendant shall handle the call out of emergency personnel.
Circumstances may require assistance for the attendant, or more than one attendant posted at different
access/entry points. The attendant(s) shall be aware of their responsibilities and be trained in the contents of
the rescue plan.
The following shall be strictly observed when performing a confined space rescue operation:
 When a person is found, or reported to have been injured or ill while working in a confined space that
has been certified safe for entry, an entry can be made by the rescue team to extricate the injured or ill
person from the confined space;
 When a person is found to have collapsed in a confined space during entry or while working, even
though the confined space has been certified safe for entry, no entry should be attempted to extricate
the collapsed person until one of the following measures have been taken:
- The conditions inside the confined space have been tested as safe for entry by an AGT or
member of the Rescue Team if he has been trained / authorized;
- The wearing of a suitable breathing apparatus and body harness when attempting to extricate or
rescue a person form a confined space;
- The presence of rescue team outside the confined space equipped with all necessary rescue
equipment for the extrication of the collapsed person.

6.11. Post Entry
6.11.1. General
After the work has completed every shift, the Performing Party shall ensure that the following steps are
completed before leaving the site.

6.11.2. Area Reinstatement


All authorized hazardous material (limited quantities) used inside the confined space to be removed.
Temporary Electric cables, and extensions shall be disconnected and removed, unless they are properly
channelled inside the confined space not creating any trip hazard or blocking the access / egress in case of a
rescue operation.
Hoses shall be removed from the confined space after completion of the shift and during any breaks such as
lunch.
Tools and other required material to execute the activity shall also be removed after the work has been
completed.
6.11.3. Closure of Entry Points
The Performing Party shall ensure that when the shift is completed and all personnel have left the confined
space and have been accounted for, all entry points shall be securely closed off to prevent unauthorized
entry.

6.11.4. Removal of Isolations


The Performing Party shall inform the PTW Issuing Authority and any Isolation Party that the work inside the
confined space has been completed and that Isolations can be removed. PTW Issuing Authority and Isolation
Parties shall complete a visual inspection and remove the Isolations as per the LOTO Standard. No PTW shall
be issued for CSE after any isolation has been removed.
6.12. Facial Hair policy
Projects that have a large number of workers with different ethnic backgrounds may require a Facial Hair
Policy.
The project Facial Hair Policy will be applicable to all personnel performing work on the project or visiting
project facilities that may require the use of a respiratory protection device. This includes Dr. Concrete
Personnel, Subcontractor employees, vendors and other visitors, who may utilize respiratory protection
devices as a function of their job, enter areas with potential respiratory hazards or have emergency response
duties.

Policy Sample:
It is the responsibility of each one to ensure that their personnel comply with all provisions of the Facial Hair
Policy.
Personnel covered by the policy as described herein must not have facial hair that prevents an effective seal
between the user’s face and the face mask of the type of respiratory protection equipment utilized at the
facility. This includes air supplied and air filtration respiratory protection devices with the exemption of simple
fibre filter masks which are primarily utilized for non-hazardous nuisance dust.
Where the effectiveness of the respirator to be utilized may be compromised by the presence of hair on the
face mask sealing region of the face, face shape/size and/or other factors, a qualitative fit test shall be
performed with the type of respiratory protection equipment utilized at the facility. Fit tests must be performed
prior to the start of work. If an effective facial seal cannot be achieved, individuals in question may not enter
designated areas of the project. In cases where personnel are required to work in areas or situations covered
by the Policy, but cannot pass the respirator fit test, they will be given an opportunity to take measures
(including clean or partial shaving in the respirator face mask sealing area) which would help to establish an

effective facial seal. If cases arise where personnel refuse to comply with the Policy as required,
Contractor/EPC Subcontractor will take measures as deemed appropriate.
6.13. Inspections and Audits
During the pre-entry briefing, the Performing Authority shall complete a Confined Space Inspection Checklist,
such as the sample checklist in ATTACHMENT-1, to confirm that all necessary preparations have been made.
The PTW Coordinator and HSE Personnel shall audit this work location utilizing their own audit checklists.

7. TRAINING
In accordance with the SHSE Training Standard, Contractor/EPC Subcontractor and its Subcontractors must
provide training to all employees who are likely to be involved in confined space entry to ensure that they
possess the understanding, knowledge and skills necessary to safely perform their assigned duties.
Employees who will be involved in the entry shall be fully conversant with the Emergency Response Standard
and have received relevant training in:
 Hazard recognition;
 Permit to Work Standards, isolation standards, purging and/or ventilation procedures;
 The use, maintenance and interpretation of gas detectors;
 The use of safety equipment and PPE, such as Breathing Apparatus (BA), respirators, retrieval harness
and lines;
 The use of communication equipment such as 2-way Radio, IS Mobile Phones, Etc.;
 Emergency, evacuation and rescue procedures (including self-rescue).
Confined Space Attendants shall demonstrate an understanding of the training and skills after successfully
completing the Confined Space Attendants course.

8. RECORDS
All documentation relevant to this procedure shall be kept in the Contractor/EPC Subcontractor SHSE
Department for reference throughout the duration of the project. Such documentation will include, but is not
limited to, the following:
 Calibration Certificates
 Rescue Equipment Inspections
 JSA for Confined Space Works
 Confined Space PTW
 Training Records
 Confined Space Inspection and Audit Records
