MEMORANDUM

SUBJECT : Investigation Report, re – Murder (Stabbing Incident)

II. MATTERS TO BE INVESTIGATED:

To conduct thorough investigation surrounding the
stabbing incident resulting to the Death of alias “Barbie” and file appropriate
charges to the suspect/s in court if evidence warrant.

III. FACTS OF THE CASE:

1. Based on Blotter Entry no. 2024-0123 dated April 2, 2024 – that on or
about 4:20 AM of April 1, 2024, SELENA GOMEZ, female, 34 years old, a
resident of Happy Homes Subdivision, Brgy Ilaya, Zarraga, Iloilo appeared and
informed this Police station alleging that a stabbing incident happened inside
Zarraga Public Market, Brgy. Ilawood Poblacion, Zarraga Iloilo. In this connection
PNP team led by PSMS Irah Morden and PSSG Cristhel Theresa Jover,
investigator on duty responded in the area. Upon arrival thereat one (1) female
victim identified only to alias “BARBIE”, female of legal age, residence unknown,
was lying under the fish stall owned by the complainant, her clothes are torn, with
blood oozing from her abdomen. Immediately thereafter municipal ambulance
was contacted and directed to proceed on the area of incident and upon arrival
boarded the victim and rushed to Metro Iloilo Hospital and Medical Center, Inc.
Metropolis Ave, Jaro, Iloilo City for immediate medical attention however, was
declared DOA at about 4:45 AM by Dr Camello, attending physician of the
hospital.

2. Initial investigation conducted disclosed that at about 3:30 AM of same date

the victim together with the suspect identified to alias “KARMA” fish vendor, a
heated argument ensued between them at the suspect’s fish stall regarding the
victim’s unpaid debts to her but the victim abruptly left the suspect back to her
fish stall. However, at about 4:05 AM of same date, due to previous confrontation
the suspect stabbed the victim with a kitchen knife she uses at work on her
abdomen and back several times which the informant, arriving to her own fish
stall, upon learning the incident pleaded the suspect to stop stabbing the victim
 but she was ignored prompting her to report the incident, and upon sensing by
the suspect that the informant sought police assistance, she immediately fled
away from the scene but was stopped on the way by JOHN SMITH, barangay
tanod. PSMS Morden and PSSG Jover arrested alias “KARMA” on the scene and
brought her to Zarraga Municipal Police Station. Recovered from scene of
incident was one (1) kitchen knife believed was used in perpetrating the incident.
The suspect was then brought to the lock-up jail of Zarraga Municipal Police
Station.

3. Based on Follow-up investigation conducted and recorded in Blotter Entry

2024-0124 dated April 2, 2024 – about 5:00 AM victim alias “BARBIE” was
identified as NICKI TANYA MINAJ, female, 41 years old (DOB December 8,
1982), single, fish vendor, resident of Brgy. Inagdangan Sur, Zarraga, Iloilo, while
the suspect identified as TAYLOR ALISON SWIFT alias “KARMA”, female, 34
years old(DOB December 13, 1989), single, resident of Brgy. Inagdangan Norte,
Zarraga, Iloilo. Victim presently lies at Morgue of Metro Iloilo Hospital and
Medical Center, Inc. Metropolis Ave, Jaro, Iloilo City.

4. Certificate of death signed at POST MORTEM CERTIFICATE OF DEATH

by PBGEN HAILEY RHODE BEIBER, MD, dated April 5, 2024 with cause of
death/notations: MULTIPLE STAB WOUNDS AT THE ABDOMEN AND BACK.

5. The complainant SELENA GOMEZ has given her Affidavit at Zarraga

Municipal Police Station at 8:00 AM regarding the killing that transpired on or
about 4:20 AM of April 1, 2024 at Zarraga Public Market, Brgy. Ilawood
Poblacion, Zarraga Iloilo.

6. The suspect TAYLOR ALISON SWIFT has given her Affidavit together with
his legal counsel, ATTY. KATY PERRY at Zarraga Municipal Police Station at
4:00 PM of April 3, 2024 regarding the killing that transpired on or about 4:20 AM
of April 1, 2024 at Zarraga Public Market, Brgy. Ilawood Poblacion, Zarraga Iloilo.

5. About 9:00AM April 5, 2024, this station represented by PSSG Cristhel

Theresa Jover, investigator on case together with the complainant SELENA
GOMEZ to Iloilo Provincial Prosecutor’s Office, Iloilo City to file a case of
MURDER.

IV. DISCUSSION/FINDINGS:
1. Based on the preceding facts and circumstances, that
about that about 4:20 AM of April 1, 2024, one (1) female victim identified only to
alias “BARBIE”, female of legal age of unknown residence was lying under the
fish stall owned by the complainant inside Zarraga Public Market, Brgy. Ilawood
Poblacion, Zarraga Iloilo, clothes torn, with blood oozing from her abdomen and
immediately brought by the municipal ambulance, and that about 4:45 AM of
same date the victim was declared DOA at about by Dr Camello of Metro Iloilo
Hospital and Medical Center, Inc. Metropolis Ave, Jaro, Iloilo City.

2. Based on the investigation of PSSG Jover, duty

investigator, that at about 3:30 AM of same date the victim together with the
suspect identified to alias “KARMA” also a fish vendor, a heated argument
ensued between them while but they were pacified prompting the victim to go
back to her stall. However, at about 4:05 AM of same date, due to previous
 confrontation the suspect stabbed the victim with a kitchen knife on her abdomen
and back several times.

3. Based on follow-up conducted on the following day, the

true name of the victim identified as NICKI TANYA MINAJ, alias “BARBIE”,
female, 41 years old (DOB December 8, 1982), single, fish vendor, resident of
Brgy. Inagdangan Sur, Zarraga, Iloilo, while the suspect identified as TAYLOR
ALISON SWIFT alias “KARMA”, female, 34 years old, single, resident of Brgy.
Inagdangan Norte, Zarraga, Iloilo. The suspect was immediately arrested on the
scene.

4. Based on POST MORTEM CERTIFICATE OF DEATH by

PBGEN HAILEY RHODE BEIBER, MD, dated April 5, 2024 with cause of
death/notations: MULTIPLE STABBED WOUNDS AT THE ABDOMEN AND
BACK named to NICKI TANYA MINAJ.

5. In the follow-up investigation, according to the suspect, the

victim was the one who tried to stab her first using the suspect’s knife laying on
her fish stall. Her killing the victim was a result of defending herself. She
speculated that the victim wanted to stab her to get away with her unpaid debts
from her.

V. CONCLUSION:
Based on the facts and circumstances narrated above,
about 4:20 AM of April 1, 2024 inside Zarraga Public Market, Brgy. Ilawood
Poblacion, Zarraga Iloilo, the victim NICKI TANYA MINAJ, alias “Barbie” was
stabbed multiple times resulting her sudden death and a criminal case of
MURDER by the suspect TAYLOR ALISON SWIFT alias “KARMA”, female, 34
years old, single.

The case is still ongoing.

VI. RECOMMENDATION:
Wherefore, in view of the foregoing, the undersigned
respectfully recommends that this case be referred to the Office of the Regional
State Prosecutor Iloilo City, Iloilo for final disposition.
