We are pleased to inform you that your confirmation as a regular employee of Fusion BPO Services Phils.
Inc. is effective 11 JUNE 2023.

As a regular employee, your working schedule will continue to be dependent on the Account/Department
you are assigned in. Now, therefore, you are entitled to other benefits such as Leave Privileges, Life and
Health Insurance Benefit subject to terms and conditions.

Based on the Company's standard policies and procedures on such matters which may be reviewed from
time to time, you shall be granted leaves, life, and health insurance based on the following scales:

SERVICE INCENTIVE LEAVE (SIL)

A. 6 Service Incentive Leave (SIL) - pro-rated depending on the month of regularization. It can be
used as Vacation Leave (Planned Time-Off) or Sick Leave (Unplanned Time-Off). Upon the turn of the
new year, the leave credits entitlement will refresh and at the start of the year, the regular employee will
receive a standard of 10 SIL. Current policy on Accrued leave credits of 0.5 (half day) will be earned
effectively a year after the regularization date and the next additional 0.5 will be gained for after every 6
months, will take effect.

For the Planned Time Off, it should be filed 7 business days ahead of the desired date pending approval
by the Account/Department Head. Herewith is the PTO application link.

The Unplanned Time Off is intended for sickness, accident, or emergency purposes – should not be
tagged as NCNS, not CWD, and is required to submit valid supporting document upon return to
work pending approval by HR/Health Department. Herewith is the UPTO application link.

B. 1 day Birthday Leave- should be availed or filed 15 days before and after the birth date of the year
pending approval by Account/Department Head

C. 3 days Bereavement Leave - can only be availed once in a year, and is applicable only if the
deceased is immediate family member; must submit a photocopy of death certificate upon return
to work
* Married Employee: Legal Spouse and Children
* Single/Unmarried Employee: Parents, Siblings
* Single Parent: Children, Parents, and Siblings

D. Other mandated government leaves such as:


*Solo Parent Leave (7 days- must submit a photocopy of DSWD Solo Parent ID)
*Paternity Leave (7 days- must submit Letter of intent and photocopy of Marriage Certificate or
SSS Allocation copy from wife)
*Maternity Leave (105 days further details to be discussed with Clinic and Accounting team)
wherein availment is on top of leave credits.
NOTE: Effective calendar year 2023, cash encashment of unutilized leave credits at the end of the fiscal
year will no longer be available. There is, however, the “carry-on leave”, wherein a maximum of 5 days
unused leave credits at the end of the calendar year will be released and for encashment together with
the employee’s last pay claimable.

LIFE AND HEALTH INSURANCE

The Company has granted you to enroll one (1) free dependent provided that the guidelines below for
dependent's eligibility will be followed.

A. For Married employees: Spouse aged 65 years old and below OR eldest to the youngest child
aged 30 days old up to 21 years old. Legally adopted children-stepchildren who are yet to be adopted
legitimately by the principal plan member are considered ineligible for enrollment.

B. For Single/Unmarried employees: Acknowledged natural children first aged 14 days old up to 21
years old, OR parents aged 65 years old and below, OR siblings, eldest brother/sister to the youngest
child aged 14 days old up to 21 years old.

*NOTE: You must submit a photocopy, or a full-screen picture of your dependent’s live birth certificate
should both of you have different surnames to support your relationship. Please send it to the company
clinic or email to health.cebu@fusionbposervices.com

C. For employees with "live-in"/common law partners: there should be no legal impediment to
marry one another. To secure a barangay certificate and send a letter of
intent to health.cebu@fusionbposervies.com.

D. For members of the LGBTQ+ community: declared partner must be SINGLE and to present a
picture of partner's valid ID and CENOMAR (Certificate of No Marriage Record). Send a letter of intent
to health.cebu@fusionbposervies.com for verification.

For those who opt to have an additional dependent (with fee), should send a letter of
intent to health.cebu@fusionbposervies.com for confirmation and signing of Authority to Deduct form
via electronic signature.

Subject title is "ADDITIONAL PAYABLE HMO DEPENDENT: <NAME OF EMPLOYEE>". In the


body of the email, please send details of the additional payable dependent
-FULLNAME OF THE ADDITIONAL PAYABLE DEPENDENT: LAST NAME/ FIRST NAME/ MIDDLE NAME
-AGE
-BIRTHDATE: (mm/dd/yyyy)
-Dependent's relationship to the employee: YOUR CHILD/YOUR LEGAL SPOUSE/YOUR PARENT/YOUR
SIBLING/ YOUR LIVE-IN PARTNER/ SAME SEX PARTNER

As for your application of the corporate life insurance package, please approach any nurse on duty so
they could discuss it to you in details.

We are giving you three days not later July 17, 2023 to enroll yourself and the dependent in our
insurance package thru this application form link provided.
