accordance with law, hereby depose and state that:

ELEMENTARY SCHOOL , Brgy. Sta. Maria Minalin Pampanga.

Teachers / Professional Board Examination for Teachers
e) Certified photocopy of Certificate of Passing
f) Certified copy of Transcript of Records and Diploma
g) Certificate of General Weighted Average (GWA)
h) Certificates of Specialized Training
i) Certified copy of the Voter’s Certification and/or any proof of residency
as deemed acceptable by the school screening committee
j) NBI clearance
k) Checklist of Requirements (ANNEX C)

3. I am executing this Certification to attest to the authenticity and veracity of all

documents submitted.
4. I understand that any violation will automatically disqualify me from the selection
process.

5. By executing this Omnibus Certification of Authenticity and Veracity of

Documents, I hereby authorize the Department of Education, Schools Divison of Pampanga
through the School Head, to verify the authenticity of the above mentioned documents.
