1.0 INTRODUCTION

1.1 GENERAL

Hibiscus Oil & Gas Malaysia Limited (HML) has awarded Tanjung Offshore Services Sdn
Bhd (TOSSB) under the contract title of Pan Malaysia Umbrella Contract for Integrated Well
Services (IWS) for PAC’s – Work Order Request #3 HML Workover & Plug & Abandonment
(P&A).

1.2 PROJECT BACKGROUND

Kinabalu Field
The Kinabalu field is currently being produced via the Kinabalu Complex which consists
of KNDP-A and KNDW-D. KNDP-A is an unmanned platform consisting of 20-slots drilling
bay with topside partial stabilization oil and gas processing facilities which was installed
in 1997 while KNDW-D is a minimum facility drilling wellhead platform consisting of 20-
slots drilling bay which is bridge linked to KNDP-A. FWS from production wells in KNDW-
D are sent to KNDP-A for processing together with FWS from production wells in KNDP-A
before being exported to PCSB Samarang-A Gas Platform (“SMG-A”) via 12” crude and
14” gas subsea pipeline respectively.

Kinabalu KNDP-A Platform Well Workover & P&A Project


P&A and workover activities at KNDP-A platform is for 2 wells, which is 119 (Workover)
& 111 (P&A). This campaign will be executed using a Hydraulic Workover Unit 502 (EMAS
502 HWU) with assistance of marine spread vessel under provisional of HML. The target
mobilization date for KNDP-A is tentatively on the 1st April 2023.

The HWU and the P&A associated equipment and services will be rig up and located on
the platform throughout the campaign.

In addition, this activity might overlap with other SIMOPs activities that happen
concurrently on KNB Complex. However, separate SIMOPs risk assessment shall be
conducted by HML and the existing HAZID shall then serve as an input to the overall
SIMOPs RA.

2.0 OBJECTIVE

The purpose of this document is to define the Terms of Reference (TOR) for Hazard
Identification (HAZID) study which shall be agreed prior to conducting the HAZID
workshop. This document also outlines the overall methodology of HAZID and risk
ranking descriptions to be used during the workshop.

The objectives of this HAZID study are:


• Identify the potential hazards on the facility that may pose a serious and
immediate harm to personnel, environment and assets;
• Identify the causes and the following consequences to the hazards being realized;
• Identify the engineering or operational safeguards that are in place to minimize
the likelihood, or the severity of the consequences associated to the hazards
being realized; and
• Where necessary, recommend any additional preventive or mitigation barriers
that can be implemented in the design to reduce the risk as to meet the
requirement of as Low as Reasonably Practicable (ALARP).

Process safety issues will be the focal point for this HAZID study in which their impacts
towards personnel, environment, and assets will be critically assessed. This study also
describes the preventive and mitigative measures for the identified potential hazardous
events. In general, HAZID study assumes that:

• HAZID is a technique to identify safety issues and is not used for technical review.
All equipment/system is well designed, manufactured, properly inspected and
performs reliably as per design intention.
• Only single failure results in hazards no double jeopardy unless credible and
agreed upon.
• Facilities are well maintained and operated as per the acceptable design
standard.

4.0 DEFINITION OF TERMS

The following terms as used in this document, assume the meaning given below:

COMPANY The party which initiates the project and ultimately


pays for its well workover & P&A WORKS, and in
this case is HML.

CONTRACT The legally binding documents for the WORKS.

MAIN-CONTRACTOR The Main Contractor appointed by COMPANY to


execute WORKS.

SUB-CONTRACTOR The contractor appointed by the

6.0 METHODOLOGY

6.1 OVERVIEW

HAZID is a structured brainstorming technique involving a multi-disciplinary Review


Team for the identification of potential hazards associated with the facility and its
corresponding operation.

6.2 REVIEW PROCEDURE

The HAZID study will be conducted in a workshop forum, employing an experienced


multi-disciplinary team led by an approved HAZID Facilitator. The Facilitator is
responsible to coordinate the discussion to ensure that all potential hazards are
identified. The appointed Facilitator is also responsible to guide the HAZID Review Team
in correctly identifying the corresponding threats, threat barriers, top events, and
consequences of the hazards as well as performing the risk ranking. HAZID Review Team
will be responsible in identifying the threats and the associated safety measures that
are already in place.
The procedure of HAZID study is summarized in Figure 6.2-1:

6.3 HAZID ASSUMPTIONS

Throughout the HAZID study workshop, the following assumptions will be adopted:
• In principle, only single failure results in hazard – no double jeopardy unless credible;
• All equipment is well designed, manufactured and properly inspected;
• Facilities is well maintained and operated with acceptable standard;
• The system will perform as the design intends; and
• Failure of safeguarding/ interlock system were not considered.

6.4 HAZID DOCUMENTS AND DRAWINGS

Prior to the workshop, some key and related technical documents and drawings are submitted
to the HAZID Facilitator for him/her to review and have an understanding of the philosophies.
As a minimum, the following list of documents / drawings will be provided and made available
for reference during the workshop:
• Scope of work
• Wells Program
• P&ID drawings
• Fishing & Milling BHA Diagram
• Hazard Area Classification as per API RP 505
• Safety Equipment Layouts & Escape Route
• Or any other documents if required

6.5 HAZID WORKSHEET

The salient points, safeguards, and recommendations during the workshop session will be
recorded in HAZID Worksheet shown in APPENDIX 2 of this report.

6.6 HAZID RISK RANKING MATRIX

Risk ranking is a qualitative approach in estimating the risk for a particular hazard. Each of the
identified hazards will be subjected to risk ranking exercise. COMPANY Risk Assessment Matrix
(RAM) will be used to rank the identified hazards based on their perceived frequencies and
consequences. The COMPANY RAM and Definitions are presented in Appendix 3. The aim of risk
ranking exercise is to categorize each hazard into the defined risk rating as depicted in tables as
in Appendix 2 and 3.

The risk levels will be estimated taking into consideration the severity of the consequences
(categorized into people, asset, environment, and reputation) and the likelihood of the scenario
to occur. The highest risk rating amongst the categories represents the overall risk rating for the
hazard. Note that the risk ranking will be undertaken twice i.e., with the consideration of
safeguards in place and after consideration of any additional safeguards that are being proposed.

7.0 HAZID WORKSHOP AGENDA

7.1 WORKSHOP AGENDA

The HAZID workshop session arrangement will be conducted in hybrid; physically and
virtually via Ms. Team application, which cater for those who are unable to attend it
physically, whereby each of all sub-contractors, vendors and marine key representatives will
be invited and participate in the workshop. Each key risk from the ISO 17776 reference will
be discussed in detail which is related to the P&A and workover operation work scope that
will be carried out in the KNDP-A platform by TOSSB.

7.2 HAZID PARTICIPANTS

The HAZID workshop will be facilitated by Third-party consultant and recording will be
performed by an appointed scriber. The composition of workshop team should represent a
wide spread of knowledge, expertise, and experience. This is fundamental to the success
of study and the team must be:
• Firmly committed to the study;
• Prepared to allocate time to the sessions;
• Prepared to play an active part in the sessions; and
• Prepared to revert on any queries or clarifications raised during the study.

Participation from various discipline from MAIN-CONTRACTOR, SUB-CONTRACTOR and


COMPANY are required for the HAZID Workshop and Review session. The following key
discipline representatives are required:
1) Project Representatives;
2) Project Engineer;
3) Safety Officers;
4) Operation Team;
5) Technical Team

7.3 HAZID REPORTING AND CLOSE-OUT

The HAZID study report will be prepared after the completion of the study. A summary of
actions from the HAZID workshop will be produced in the report. The summary will contain
all relevant information to provide a complete and quick reference for source of a proposed
action.

A COMPANY-approved (HML) HAZID worksheet and PHA-Pro software to be used to record


HAZID workshop sessions. HML HAZID Worksheets can then be exported to Word or Excel
if necessary. Data should only be exported following tidy-up of the worksheets by
the scriber, and approval by the leader. Also, if there are comments on the worksheets
after the report has been issued, the PHA-Pro files need to be updated accordingly for future
reference.

A sample of COMPANY-approved (HML) HAZID worksheet layout is given in Appendix 2 -


HAZID Work Sheet.

The discussions of the HAZID workshop are captured by the HAZID Scriber. Care must be
taken to note that the names and expertise of team members and participants should be
recorded, including specialist personnel who attend for a limited time. Attendees can be
classed a full or part time in the report.

The actions arising from the HAZID study need to be followed up to ensure the nominated
action party is accountable for the implementation and close-out. The actions are to be
included in the HAZID Close Out Report or equivalent for tracking and monitoring. (Ideally
only one party for each recommendation / close out). CONTRACTOR / VENDOR shall issue
the close-out report, which will contain correspondents’ comments, evidence where
required (i.e., report, drawings, datasheet & etc.), as well as confirmation that issues have
been resolved, mitigated, or become not applicable.

APPENDIX 1- HAZID GUIDEWORDS

ISO 17776 [1] provides general guidance on tools and techniques for hazard identification and risk
assessment in offshore oil & gas production facilities. This document also presents a
comprehensive hazard checklist for identifying different hazards which may be associated with
offshore O&G production activities. The original list in the standard covers all types of hazards,
including major accident, flammable, workplace and security hazards. Since the majority of HAZID
studies undertaken by ERM will be concerned with major accidents, the checklist has been edited
to show only those hazards associated with major accidents or flammable hazards.

This type of checklist only provides general high-level guidance on the types of hazards
encountered and the workshop will need to explore the detail of, for example:

• how this hazard category is present in a particular node,


• the possible harmful effects of that hazard,
• what are the possible causes of any hazardous event,
• whether there are any known prevention or mitigation measures in place for this hazard.
