AFFIDAVIT

I, CHERRY B. ENRIQUEZ, of legal age, Filipino, married and a resident of

Brgy. East, San Jose, Northern Samar, Philippines, after having been duly sworn
to in accordance with law, depose and say:

1. That I am not the true, absolute and registered owner of the house located at
Barangay East, San Jose, Northern Samar but I am the occupant of the said
building.

2. That the fight between Jasper Tadeo and Marc Angelo Decena, which
happened on August 5, 2023, caused damage to the water pipe of the said
building. That the photos evidencing the damage are hereto attached for
reference;

3. That I am executing this affidavit to attest to the voracity of the foregoing, and to
claim insurance benefits for the repair of the aforesaid water pipe, and for
whatever legal purpose this may serve.
