I am Giah Doreen B. Mesiona, and I am writing on behalf of my thesis groupmates, Marc Dave
Setiar and Getonriel Rodico Rosales. We are currently enrolled as fourth-year Bachelor of
Science in Management Accounting at Central Mindanao University. As part of our
undergraduate thesis program, we are undertaking a study entitled “Effects Of Internal Control
System And Human Resource Competence To The Financial Accountability Of The Local
Government Units (LGUs) In a Province In Mindanao.” Our research aims to examine how
internal control systems and human resource competency affect the financial accountability of
local government specifically in the Province of Bukidnon.

In connection with the development of the said study, we are humbly asking your good office to
permit us to conduct our data gathering through a survey questionnaire. The target respondent for
this questionnaire is Municipal Accountant.

The undersigned intends to utilize such data as basis for identifying the study’s respondents, a
critical factor in the commencement of the data-gathering process. Rest assured that such data
