NARRATIVE REPORT

As the days progressed in Wesleyan Hospital, I found myself gaining more confidence and

becoming increasingly comfortable in my role. The clinical exposure I was receiving was

invaluable, and I was eager to absorb as much knowledge as possible.

September 18, 2023 marks the first day of our duty in Wesleyan Hospital. It was all new to me

since this is my first clinical exposure as well. It is hard that I finished my 3rd year college with

only pure online classes and my batchmates are already familiar with handling patients but I

managed to cope up with them somehow. Our shift is from 7 am to 3 pm in the afternoon; I

arrived 20 minutes prior to duty. We waited for the head nurse to give us a heads-up for the

endorsement. After that, the head nurse assigned us with patients we’re going to manage during

their whole stay in the hospital. Since it was my first time, Ma’am Alex (one of the head nurses)

taught me the charting. She discussed every part and what is the use of each forms and sheets.

My patients are 2 pedias with the diagnosis of AGE with moderate dehydration. I learned to

regulate an IV, prepare the medications such as ampules and vials, and administered it to the

patient.

On Tuesday, September 19, under the guidance of Ma'am Alex, our day at the hospital began

with an instructive lesson on the calculation of inotropes and their concentrations.

Simultaneously, I diligently organized the patient's chart, as it was the day of their anticipated

discharge. Our responsibilities for the day encompassed providing attentive bedside care and

meticulously monitoring vital signs.

Jumping forward to September 25, my duties took an exciting turn as I welcomed two new

patients into my care. One patient presented with a diagnosis of URTI, while the other faced the

challenging condition of protein energy malnutrition combined with the added complexity of a

high-risk brain tumor. Throughout the day, I remained in a state of continuous observation,

shadowing the experienced nurse on duty. I seized every opportunity to seek information about

unfamiliar aspects of medication administration and the intricate preparation of a 3-way stopcock

with a male lock adapter. Additionally, I actively assisted in the removal of an IV from one of

the patients. As our focus primarily revolved around ward duty, meticulous charting became our

top priority, supplemented by the routine tasks of recording intake and output.

On October 2, 2023, I was tasked with the care of three patients during my inaugural clinical

duty. The first patient required immediate surgery due to acute appendicitis, while the second

patient presented with alcohol intoxication, hypertension, and CAD. The third patient had PCAP.

Throughout the day, we diligently monitored their vital signs every four hours as part of our

routine. Undoubtedly, it was an exceptionally challenging day for me, especially given that it

marked my first clinical duty. Managing three patients simultaneously while also being

responsible for charting, as a student nurse, was a formidable task.

During the last day of our duty. October 3, 2023. We just did the usual monitoring of patients

and charting. It was a lot lighter compared to the other day. . Along the way, I also acquired new

skills, including the preparation of the volumetric chamber. As the clock ticked closer to the end

of our shift, we couldn't help but reflect on the valuable lessons we had learned during our time

with the nurses. Their guidance, support, and expertise had been instrumental in our growth as

healthcare

professionals. We shared stories and laughter, creating a warm and friendly atmosphere to

express our gratitude. It was a bittersweet moment, knowing that we would soon be parting

ways, but we left with a deep sense of appreciation for the bonds we had formed and the

knowledge we had gained. We continue learning from their experiences even after our duty had

ended. as the day unfolded, I found myself gaining invaluable experience. This experience

illuminated the vital and multifaceted role of a nurse in healthcare. I am profoundly grateful for

the unwavering support and guidance provided by Ma'am Ena and Ma'am Alex. They were not

only accommodating but also remarkably hands-on in their approach. Their dedication to

ensuring that we, as student nurses, fully experienced the everyday life of a nurse was truly

commendable.

In conclusion, my first clinical duty was undoubtedly challenging, but it served as a profound

learning experience that reinforced the significance of nursing in patient care. The mentorship of

Ma'am Ena and Ma'am Alex played an instrumental role in my growth and understanding of this

noble profession. I look forward to continuing to build on this experience as I progress in my

nursing journey.
