MARINE DECK OFFICERS DIVISION (MANILA) DOCUMENTARY REQUIREMENTS FOR ADMISSION TO THE OFFICER-IN-CHARGE LICENSURE EXAMINATION (WRITTEN PHASE)

(EFFECTIVE JANUARY 2007) FIRST TIMERS 1. Properly accomplished Action Sheet (PRC MDO Form No. 03). Please use black ball pen in filling up forms. 2. Original and photocopy of the Ship Simulator and Bridge Teamwork with BRM (SSBT) & Assessment Certificate. 3. Original and photocopy of BSMT Transcript of Records with Scanned Picture, date of graduation (mm/dd/yy), S.O. number, seal of school and remarks for licensure examination purposes only. 4. Original and photocopy of Authenticated Birth Certificate in Security Paper (SECPA) issued by the National Statistics Office (NSO). 5. Training Record Book (Per Memo Circular No. 2000-03 Series of 2000). Original and photocopy of all entries on Training Record Book (log book). 6. Original and photocopy of NBI Clearance valid prior to the first day of examination. 7. Original copy of valid Medical Certificate (PRC MDO Form No. 04) or Original and photocopy of valid Comprehensive Medical Certificate issued by DOH accredited Medical Clinics and hospitals. VALIDITY: Medical Certificate (PRC MDO Form No. 04) must not be more than six (6) months from the date of issuance prior to the first day of scheduled examination OR Comprehensive Medical Certificate must not be more than one (1) year from the date of issuance prior to the first day of scheduled examination. 8. Original and Photocopy of Community Tax Certificate / Cedula (current year). 9. An approved Sea going Service as Apprentice Mate / Deck Cadet / Ordinary Seaman / Able-bodied Seaman for at least twelve (12) months on board sea going ship of 500 Gross Tonnage or more. Proof of Sea Service: (9.1 9.4) 9.1 Original Certificate of Sea Service (PRC MDO Form No. 05) 9.1.1 Properly accomplished all data filled in 9.1.2 Certified / Signed by the Master of the vessel 9.1.3 Attested by the ships company or agency 9.1.4 Duly Notarized 9.2 Original Certification from shipping or manning company stating the name and type of vessel, gross tonnage, trading area, rank occupied and inclusive dates of sea service. 9.2.1 Signed by the president / Manager / Authorized signatory of the company. 9.2.2 Affixed company dry seal 9.2.3 Duly Notarized 93. Authenticated copy/ies of Approved POEA Contract of Employment with date of deployment to confirm existence of the ship stated in the certificate of sea service (for foreign trade) OR Notarized Crew List from the vessel stamped by the Philippine Coast Guard (for domestic trade.) 9.4 Original and photocopy of Seamans Identification Record Book (SIRB) 9.4.1 Photocopy of pages 1,2,& 3, pages indicating sea service to be submitted and arrival & departure stamped by the Philippine Immigration.

10. Four (4) pieces Passport size colored picture: white background, in Proper Merchant Marine uniform with one (1) gold stripe, no head gear, no eye glasses and with complete name tag, (DELA CRUZ, JUAN JR. SALVADOR). 11. One (1) mailing envelope with metered mailing stamp. Please pay at the Customer Service Center, Ground Floor. 12. Examination Fee: PHP 900.00 (first timer and repeater) PHP 450.00 (conditioned) Note: For those taking the nullified subjects please present your original official receipt previously issued in the June 2002 Examination. 13. Metered Documentary stamp for (CAF). Please pay at the Customer Service Center, Ground Floor. * ANY DEVIATIONS FROM THE ABOVE MENTIONED REQUIREMENTS ARE SUBJECT FOR INTERVIEW OR TO SUBMIT OTHER REQUIREMENTS AS REQUIRED BY THE BOARD OR MARINE DECK OFFICERS. REPEATERS / CONDITIONED / NULLIFIED: To submit item nos. 1,3,4,6,7,8,10,11,12,13. Applicant who graduated under the OLD BSMT curriculum and wants to avail exemption from taking practical assessment may submit item no. 2. WARNING: Submission of fake / fraudulent documentary requirements is administratively and criminally liable under R.A. No. 8544. IMPORTANT: All application for licensure examination must be submitted personally by the candidate at the Marine Deck Officers Division (4th Floor, PRC Annex Bldg. P. Paredes St., Sampaloc Manila) or at the PRC Regional Offices (Baguio, Cagayan de Oro, Cebu, Davao, Iloilo, Legazpi, Lucena, Tacloban, Tuguegarao and Zamboanga) on or before the deadline. REMINDER: Three (3) Times Failed Examinees are required to rest for one (1) year. (Section 23 Article V of Board of Marine Deck Officers Resolution No. 02-04 s. 2002 dated February 1, 2002, revised implementing Rules and Regulations Implementing RA 8544 the Philippine Merchant Marine Officers Act of 1998. NO APPLICATION SHALL BE ACCEPTED AFTER THE POSTED DEADLINE.
