1. GREETINGS AND INTRODUCTION
Good morning everyone! Welcome to TESDA CSITE KALINGA, accredited assessment center in
________________ NCII.

My name is _____________, I am your accredited competency assessor.

2. ACKNOWLEDGEMENT
I would like to acknowledge the presence of our TESDA Representative Ma’am IMEE ANN T. DONG-AS.
Do not worry about her presence because she is here to oversee the conduct of the competency assessment
and check if the conduct is in accordance with the prescribed competency assessment, methodology and procedures.
They will not assess you.

3. CHECK ATTENDANCE AND COLLECT ADMISSION SLIP

Before I start, let me first check the attendance. As I call your name, please raise your hand, come in front and present to
me your admission slip then affix your signature in this attendance sheet.
