LETTER OF EXCUSE

Good Day,

As the school nurse of this institution, I am writing to request permission for absence of classes of Jeanathan N.
Mamalias and Beatrice C. Camocamo of Grade 9 Kingbird under advisory of Rizza Noval due to school-related
activity. The activity is an essential part of their educationla experience and requires their participation as Clinic
Response Unit (CRU) of Digos City National High School.

The purpose of this school activity is enhance and develop their skills in responding emergency situation of the
school and alleviate suffering of the learners in times of natural disaster. I believe that equipping young minds
with theses skills is not only benefical for their personal growth but also significantly contributes to the welfare of
the community.

I understadn the importance of regular attendance and will ensure that they will makes up any missed assignments
or classwork. We also promise to keep any disruption to a minimum and will ensure that they’ll catchh up with
material the miss during their absences.

I, kindly request your permission for Ms. Mamalias and Ms. Camocamo to participate in this engagement. Please
let us know if there are any specific procedures or assignments they need to follow during their absence.
