UNDERTAKING

During the process of filling of entry level application for NABH it was found that the following documents
were not available with our hospital. I undertake that these documents and any other document that is lacking in
the application would be prepared in due course of time and submitted for your kind consideration.

1. Copy of the Blood transfusion committee's minutes, discussed reaction and CAPA..
2. Process addresses discharge of all patients including Medico-legal cases and patients leaving against
medical advice.
3. Documented policies and procedures are used to guide the rational use of blood and blood products.
4. Documented procedures govern transfusion of blood and blood products.
5. Documented procedures address procurement and usage of implantable prostheses.
6. Documented policies and procedures govern usage of radioactive drugs.
7. Documented policies and procedures for maintaining confidentiality, integrity and security of records,
data and information.
8. Define process to whom the patient record can be released.
