PURPOSE: To ensure that the pain clinic runs smoothly, and patients are seen at a timely manner.

PROCEDURE:

1. Referrals will be sent to the pain email.


2. The nurse is responsible for calling the patient and scheduling them for the appropriate day and time to
see Dr. Hussein or Rebecca Ultch, PA.
3. Once an appointment is made, a chart is the next thing to do.
4. All follow-up appointments or acute illness visits should be brought to the nurse’s attention before
scheduling the patient for an appointment. This ensures that timely care for the appropriate provider is
provided.
5. Included in the chart should be the following:
a. Last relevant dictation. (this should be printed twice, one for Rebecca that Dr. Hussein will use for
his dictations, and one for the charts)
b. Any imaging or updated imaging relevant to the injury/concern.
c. Any corresponding notes from outside providers/referrals (EMG’s/neurology consults/ED
visits/etc.).
d. Pain assessment form on the front of the chart to
e. An updated medication, allergy, and diagnosis list also included in the front of the chart.
6. Callbacks will be done for the corresponding day the following week.
7. The nursing staff from each office will be responsible for doing callbacks a week before patients appt.
8. During the call back the nurse will remind patient of appointment date and time and remind patient of
any testing that needs to be done prior. Designated staff will also make sure to get the location of where
that testing is getting done and if that testing has been scheduled yet.
9. The nurse will document in detail all callbacks in nurses’ notes.
10. The pain/spine receptionist will then call the patient the day prior to their office visit to confirm the
appointment for the next day. Once confirmed they will mark appointment as confirmed in Intergy.
11. Once the patient has arrived in Medical Arts, the patient will check in at the front desk.
12. The nurse will then check them in, asking them about their concerns/injuries, obtaining vital signs, going
over medications/allergies, and having them rate their pain.
13. If they are in the office for a follow-up visit, then the nurse needs to assess how the injection worked for
them. This is also to include a reduction or increase of pain.
14. The nurse will then escort them into the Neuro suite and place them in a room. Once they are done with
that patient they move on to the next patient, etc.
15. The nurse is responsible for sending off any outpatient testing orders and surgery scheduling sheets that
are needed for each patient.
16. The nurse is responsible for ensuring that all follow-up appointments are made for the appropriate
provider.
