with your office to undergo WORK IMMERSION for 80 hours as a requirement of the

order to update, upgrade and make the education of our students more relevant to the actual

Sir / Ma’am:

This is to request permission from your good office to please allow Christian Rey
Tolero a Senior High School - Grade 12 student, to undergo a 80 hours of WORK IMMERSION
in your company/agency to acquire industrial ORIENTATION. The training period will start
immediately upon your acceptance of the bearer under the terms and conditions as stipulated in
the duly accomplished memorandum of agreement hereto attached for your confirmation.

Moreover, a student evaluation is provided where a student is to be assessed on his/her


performance, attitude and behavior to be attested by the supervisor. The Immersion Teacher and
Trainer handling each field of specialization from our school will visit your establishment from
time to time to find out how the student perform in the working area. Thank you very much for
your usual cooperation and assistance extended to the school

Introduction (1 to 2 pages)

The introduction contains a brief background regarding the work


immersion in the K to 12 Curriculum and its importance, application or
impact to the grade 12 students.
Objectives of the Work Immersion ----The student enumerates
what are to be expected from them at the end of the work immersion
Duration and Place of Work Immersion---The student writes down
the place and duration of the work immersion.

The Work Immersion Venue (minimum of 3 pages)


Mission (Write the mission of the training area)
Vision (Write the mission of the training area / office.)
Organizational Structure. (Describe the organizational structure of
the office to where the student was assigned for her / his
work immersion. The photo will be included
The Training Experience
 The student will write a paragraph that will be a brief
description of the work immersion.
 A daily narration (insight, observation, reflection of the
experiences of the student as a trainee. (from day 1-10)
 Photo documentation supporting your daily experience with
caption.
 Each day will be separated by writing the date and day.
