ANNEX “B”

AFFIDAVIT OF SELF-ADJUDICATION

I, MATEO F. GIDON, of legal age, Filipino, married and a resident of Makati City, after
having been duly sworn to in accordance with law, depose and state:

1. That I am the only child of GLENNA F. GIDON who died without any last will and
testament on November 30, 2019 in Sexmoan, Pampanga as evidenced by her Death
Certificate issued by the Municipality of Sexmoan hereto attached as Annex “A” and made
an integral part of this Affidavit;

2. That at the time her death, my mother had no debts, liabilities or obligations to any persons,
agency or institution;

3. That the only properties she left are: A house and lot, consisting of 80 square meters located
in Mandaue, Cebu City worth P75,000,000.00, representing her 50% share in the co-owned
real property, more particularly described in TCT No. 54321 hereto attached as Annex
“C” and made an integral part of this Affidavit; a BMW 1-Series car, year model 2010, plate
number TAS-013 and cash deposits in the Bank of the Philippine Islands with Savings
Account 207-20389, Banco De Oro with Savings Account 897-39432 and Metrobank with
Savings Account 233-556762, amounting to a total of P100,000,000.00 ;

4. That pursuant to Rule 74, Sec. 1 of the Rules of Court, I hereby adjudicate unto myself the
above-described real estate by means of this Affidavit and hereby files the same with the
Register of Deed of Makati City with the request that the said adjudication be made effective
without judicial proceeding as prescribed by the aforementioned provision of the Rules of
Court.
