1 SCOPE
The plan will also serve as guidance when briefing and awareness external organizations, such
as the fire brigade and ambulance services that may render support to an emergency affecting
the Project.
This Emergency Response Plan (ERP) is valid for the initial construction phase of chimney and
shall be reviewed annually (or more frequently as necessary) to ensure it is appropriate for use
as new activities and work areas open up. For the pre-commissioning phase, the Emergency
Response Plan will require a very specific update.
This document applies to all site construction activities to be performed within all Project
Construction areas, personnel involved, and covers Health, Safety, and Environmental
emergency aspects for the duration of the work.
This Plan will be distributed to TPL CONTRACTORS but also to the Project Managers /Site
Manager and HSE focal points of all TPL CONTRACTORS involved in construction activities as
these may become involved in supporting emergency response activities and who shall also
play a role of briefing all workers on site on how to react in case of emergencies.
1.1 Purpose
This Emergency Response Plan (ERP) has been developed to outline the necessary
emergency measures to be considered, planned, implemented, and monitored on the Project
for all types of emergencies, which could occur in any area of the Project chimney construction
working areas.
The ERP also takes into consideration the project works that will be conducted in areas with in.
Evacuate the Project site and report to the designated Assembly points carry out a head count
and report for further information.
This plan serves as a guideline for all personnel in the Project to observe and follow in the event
of an emergency such as:
Fire/Explosion
Medical Emergencies, including fall from height,

In the event of any unanticipated major emergency arising from the above eventualities, every
effort shall be explored in an effort to obtain the assistance of local public authorities, through
CGPL to provide for an orderly and legal evacuation.

The following terms used in this document have the meaning defined below:
CONTRACT The Form of Agreement together with the documents in order of priority,
including the exhibits, drawings, specifications and documents referred
To and in the order of precedence listed in the Form of Agreement.
PROJECT Means the development, financing, design, engineering, manufacturing,
procurement, installation, construction, testing, pre-commissioning and
commissioning.
SCOPE OF WORK The scope of work to be performed and services to be rendered by
CONTRACTOR under the CONTRACT in relation to the realization of
the PROJECT.

Site Management has the responsibility to ensure the project is staffed, and personnel are
trained, and equipped for incidents Emergency Response.
He is also responsible for contacting site representative, and ensuring that equipment
are available for 24-hour emergency response.
At the designated Emergency Control Centre (CGPL Control Room) to receive
information and direct the whole operation of evacuation.
He shall give order for the evacuees to return to their work areas after the emergency
situation has been kept under control where this is deemed necessary.
The HR/ADMIN Manager will be responsible for ensuring that there is 24-hr coverage in the
office or designated accommodations to answer emergency calls and to call out HSE Manager,
who will advise key personnel as indicated on Appendix 1.
Responsible in the execution of first aid treatment of injured persons in the emergency first aid
clinic and to arrange and transport to CGPL Occupational Health Centre (OHC) (or other
designated clinic/hospital) for further treatment depending on their condition.
Responsibility of Employees
Employees shall evacuate from the work place in an orderly manner and assemble at their
working designated Assembly Point (near the project site office and project entry gate).
5. INSTRUCTIONS
The following Emergency scenarios are taken into consideration in the present ERP:

1. Emergency Declaration:-The siren shall be wailing full sound for 30Seconds-a gap
of 5Sec. The sequence is run for 3 times.
2. All Clear:- For all clear situation, the siren shall be straight run sound for 2
minutes.
3. Testing of Siren:- For testing (Usually done on Tuesday 12:30 hrs.), the siren shall
be straight run sound for 1 minute.
immediately stop work, all vehicles stopped, parked off the road and turned off, leave vehicle
keys in the ignition, shut down any running equipment (welding machines, grinders, portable air
compressors etc.) and walk swiftly to the nearest assembly point.
The Site Manager will issue the instruction to return to work when he has ascertained that it is
safe to do so or, in case of emergency arising from existing installations, when being informed
by CGPL Incident Controller (IC) that the area is “All Clear”.
The HSE Manager will disseminate this instruction to the HSE representative for action and
further circulation.
This paragraph details the procedure in case of emergency situations arising from existing
installations.
In the event of hearing an alarm siren from the adjacent operating Plant and/or existing
installations, being aware of toxic gas release, fire emergency or being faced with an imminent
source of widespread danger, employees shall raise the alarm and inform their direct Supervisor
to contact the CGPL Emergency Control Centre with brief details of the emergency.
The following are general instructions given to TPL personnel and workforce:
Once adjacent operating plant emergency siren is activated, TPL personnel and
workforce will stop immediately all works and workers will evacuate the area. Work can
only resume once declare the area is safe and that TPL can do so.
All PTW(s) will be ceased and revalidation from area authority is required to resume the
work.
Upon notification of the external emergency arising from existing installations the control centers
will inform the Key Personnel on Appendix 1 (including the Doctor / Nurse) and ambulance that
will remain on “Stand-by Alert”.
All workers shall cease work, shut down engines of vehicles and switch off electrical equipment
before proceeding in an orderly manner to the designated Emergency Assembly.
Mobile telephones must be provided / distributed by TPL to their respective Supervisors and
HSE staffs for linkage/coordination purpose in an emergency situation.
Hand-held “Loud Speakers” will be provided by TPL for each work location to assist its HSE
staffs in spreading the awareness of an emergency situation.
The safe evacuation of personnel from each remote location shall be considered and as
necessary a suitable means of transport and driver shall remain with the workers to ensure that
their rapid evacuation to their designated Assembly Points.
Wind direction socks are installed at various elevated locations around the construction site to
provide visible identification of wind direction.
An overall evacuation of the site and/or camp shall be coordinated by the Supervisors who shall
be made fully aware of the detailed evacuation procedures.
TPL Supervisors will also check that their areas have been successfully evacuated and workers
have proceeded to the assigned assembly Points.
It will be the responsibility of the site manager to ensure that all the personnel in their respective
areas are aware of the emergency situation arises.

2nd STEP
The TPL Supervisor will contact the Emergency Control Centre (CGPL Fire Station), informing
about the event occurred and if medical assistance is needed to be contact CGPL Medical
Center.
In notifying the Emergency, the language used by TPL Supervisor shall be HINDI spoken
SLOWLY and CLEAR.
The Supervisor shall report:
Name and Company
Type of event
Exact location (unit/equipment, street) of the incident
Time of the accident/incident
Whether medical assistance is needed or not
Is nearby equipment threatened, if yes which one?
Request if the message need to be repeated to ensure understanding and
acknowledgment
In the meanwhile, initial attempts are made to put out the fire with available fire extinguishers
and if available, the use of the fire water main.
3rd STEP
Once being informed, the HSE Manager will visit/attend the incident and will evaluate the
progress of the emergency; in this phase the HSE Manager will give the initial instruction of
response activities.
The HSE Manager shall visit/attend the location of the incident, giving all support needed to the
HSE Manager.
In case of Minor Fire, all works activities and construction machinery / equipment shall
be stopped nearby the affected area.
All personnel with no emergency duties shall leave the affected area and keep away it
until the “All Clear” signal has been given.
Members of Site Emergency Response Team shall extinguish the fire immediately using
appropriate firefighting equipment, if considered safe to do so. If practical, operate the
nearest fixed fire extinguisher monitor to extinguish the fire or control the surrounding
equipment.
When the fire has been extinguished and the “All Clear” signal has been given by the
Incident Controller, all personnel shall return to their respective work site.

In the event the fire is a Major Fire “out of control”) and cannot be safely extinguished
by emergency team, the HSE Manager will contact the client and will require local
assistance by calling and stating:
o Name, company and telephone number
o Location and extent of fire
o Fire-fighting activities ongoing
In addition to the Minor Fire, in case of a Major Fire the following actions shall take
place:
o All personnel with no emergency duties shall evacuate the site safely to their
Assembly Points
o Personnel with emergency duties will mobilize in accordance with their respective
roles and rules already described in this document.
o Incident Controller will ensure that safe access route for emergency vehicles
shall be kept free of obstructions and one member of ERT shall open the road
barriers.

4 STEP
Once the fire has been extinguished, the “All Clear” signal has been given, all personnel at the
assembly points shall return to work site safely after revalidation of the PTW.
Site management and Emergency Response Teams shall isolate the incident area and make
further investigations.
A report of the accident/incident will be prepared by TPL Project Manager and submitted to
according to the specific procedure “Reporting and Investigation of Accidents and
Incidents” to CGPL within three days and telephonic information to security control room
and FGD Project manager.
Note: Fire and emergency action/instruction notices shall be posted on the job site, in the office
area and at the accommodation camp.
1st STEP
The fellow employees shall raise the alarm by informing immediately the nearest TPL
Supervisor or the HSE/Safety representatives on site and CGPL Security Control Room.
2nd STEP
The TPL Supervisor or the HSE/Safety representatives will contact the Emergency Control
Centre informing about the event occurred and if medical assistance is needed.
In notifying the Emergency, the language used by the Supervisor shall be HINDI spoken
SLOWLY and CLEAR, stating:
Location of the incident
Extent of injuries

Description of symptoms
Seriousness and nature of the emergency
Name and employer of all injured parties
In this phase the Site Manager will act as the Incident Controller (IC) and chief of emergency
operations.
3rd STEP
On being informed, the HSE Manager will visit/attend the incident and will evaluate the progress
of the emergency.
HSE Manager shall visit/attend the location of the incident, giving all support needed.
Depending on the severity, the Incident Controller will decide if the Site Doctor and the
ambulance are needed at the incident location.
4th STEP
The Site male nurse shall appraise the situation and in the event of need, will request
assistance from the nearest medical facility (CGPL Medical Center) or other designated
clinic/hospital) giving details of the emergency:
Location;
Type of accident, including symptoms and extent of injury(s);
Name(s) of injured person(s);
Caller’s name and telephone number;
Arrangement will then be made to meet the responding ambulance crew at the Site entrance as
appropriate.
Site Manager will then assess the situation and determine if any other measures such as
informing outside authorities are necessary.
Once the emergency is under control and the “All Clear” signal has been given, all personnel
shall return to their respective worksite after revalidation of the PTW.
Site management and Emergency Response Teams shall isolate the incident area and make
further investigations.
A report of the accident/incident will be prepared by TPL Project Manager and submitted to
according to the specific procedure “Reporting and Investigation of Accidents and
Incidents” to CGPL immediately and telephonic information to security control room and
FGD Project manager and Head Safety CGPL.

of medical emergency, fire/explosion, and gas release shall be as per sec.7.2, sec 7.1 and sec
7.3
Rescue equipment to be used by competent personal includes Man riding basket, Stand
by mobile crane and emergency structure basket.
Personnel are appropriately trained in height rescue and first aid.
The rescue plan should enable the victim to be removed from height as quickly as
possible to prevent the developing suspension trauma.
Rescue teams that are adequately trained and equipped to perform work-at-height
rescues are designated and that can respond in a timely manner.
Once removed from height individuals trained in CPR will begin to assist injured
employee. (If required)
Electrical injuries
Contact with a live conductor may cause serious external and internal burns, serious
internal organ
Damage and heart fibrillation. Also a person close to a large electrical arc may suffer
serious burns
From the heat and ultraviolet rays. Ultraviolet rays may also cause flash burns to the
eyes in the same way as an electric welder
Any person receiving an electric shock that causes any external burns, any incapacity or
any other
Symptoms must have urgent medical attention.
Seek medical treatment immediately.
7.11 Covid-19

Before Re-opening:
Disinfectant spray & Pest control in all respective work area, site office, camp, employee
transport vehicle, store area and wash room has to be followed as per guidelines.
Adequate stock of PPE’s like Face mask, hand sanitizers/ liquid soap, hand gloves &
Infrared-thermometer.
Adequate & uninterrupted drinking water and hand wash point with adequate Liquid
soaps for washing hands (tiffin, lunch and dinner) in work area/ site office as well as
workmen camp has to be ensured.
Adequate alcohol based hand sanitizers in site office & work area has to be ensured.
Isolation/Quarantine area to be maintained
Qualified Male nurse available at site for monitoring health & hygiene.
Upon Re-opening:
Staff / worker physically fit are only permitted to enter site. (Self-Diagnosis required)
If any found with Cough, shortness of breath, sputum / sore throat & muscle pain are
restricted to enter site.
While entering through Gate -2 all has to follow min 1 meter social distancing for safe &
smooth worker movement.
While entering gate it is mandatory to undergo “Body Temperature check-up” using
Infra-Red Thermometer & the same has to be recorded in register for 3 times a day as
follows
I.e. Morning at Gate Entry
Afternoon after lunch before starting work
Evening while leaving site
Without standard Face mask & hand gloves no staff / workers are allowed inside site, so
mandatory PPE’s includes
Safety helmet
Reflective jacket
Safety shoes
Standard Face mask
Hand gloves
During working hours at site social distancing (min 1 meter) has to be followed without
any deviation. ( Includes both staff & worker )
No new staff / workers allowed to enter site till further instruction.
Only staff & workers available in Nellore district shall be permitted to enter site till further
instruction.
If any staff / worker willing to resume duty have to take prior permission from TPL Site
management and has to undergo Quarantine period as applicable before starting the
work.
Employees to undergo one hour awareness session before starting their work.
All workmen to undergo Refresher Safety induction training before starting the work.

After Re-opening:
Checklist for start of work after Covid lockdown has to be ensured on daily basis.
While doing these works all the safety precautions are taken to minimize injury to personnel and
material damage at all levels of the chimney.
It is anticipated estimated that the maximum of 40 personnel including SLIPCO/TPL and CGPL
may well be on the top levels of the structure at any given time. The passenger lift is certified to
carry 5 personnel/600Kgs.
It is suggested that in an extreme emergency such as a fire, the use of the material and or
concrete buckets may be used. There are one concrete buckets and one material lift certified for
materials only but to a higher weight capacity than the passenger lift.
Personnel must remain inside bucket or materials basket, must be wearing harnesses and
attached at all times on descent.
Personnel on the horizontal platforms will work behind firmly affixed barricading on all edges of
the structure sides. This barricading will be supplemented with the provision of a safety lifeline

to so personnel can attach the safety harness lanyard hook to avoid any falls from height and
therefore to minimize accidents.
In case, a worker suffers from any illness or an incident or any sort of trauma, the information
will be provided by the site supervisor to all concerned:-
Suspension trauma can be influenced by all aspects of the fall, so they are all equally important.
Rescue must be assured within five minutes of the occurrence of a fall to minimize the risk of
further injury or death due to suspension trauma.
When a worker falls and is suspended in a harness, it’s important to rescue him or her as
quickly as possible because of the following reasons.
• The worker may have suffered injuries during the fall and may need medical attention.
• When workers are suspended in their safety harnesses for long periods, they may suffer
from blood pooling in the lower body. This can lead to suspension trauma.
• Suspended workers may panic if they are not rescued quickly.
• The event that led to the fall may create additional risks that need to be addressed.
Fall victims can slow the onset of suspension trauma by pushing down vigorously with the legs,
by positioning their body in a slight leg-high position or by standing up.
Harness design and fall injuries may prevent these actions.
Fall Arrest the whole concept of fall protection is that operatives who fall will be stopped
by a tethering system.
Rescue must come rapidly to minimize the dangers of suspension trauma.
In situations where self-rescue is not possible, operatives must be supervised at all
times. Regardless of whether an operative can self-rescue or must rely on others, time is
of the essence because an operative may lose consciousness in only a few minutes.
For conscious casualties where possible, those suspended keep their legs moving to
keep the blood pumping and reduce the risk of venous pooling,
If a worker falls and is suspended by a safety harness, implement the emergency response plan
by following the steps below.
1. The site supervisor (or alternate person) takes control of the situation.
2. The site supervisor raises the emergency situation by Walkie talkie or mobile.
3. All workers in the immediate vicinity of the incident stop working.

4. The site supervisor quickly evaluates the situation and identifies any further hazards
that could arise.
5. The site supervisor or their designate goes to get help if workers are close by. If no
one is close enough, the site supervisor calls for help.
6. The site supervisor calls number below to notify TPL Emergency team, ambulance and
CGPL Emergency services.

7. The winch operator remains on standby and waits for further direction in case the
passenger cage is required, and the passenger cage shall be returned to the ground to
transport the emergency medical team to the top of the chimney.
8. The site supervisor (or a worker assigned to the task) isolates the accident zone and
its perimeter to limit further exposure.
9. The site supervisor (or a worker assigned to the task) moves all non-affected
personnel to a safe zone/nearest assembly point or directs them to remain where they
are.
10. The site supervisor enables Walkie talkie radio silence on the job site, except for crisis
communications from emergency responders.
11. The site supervisor sends a designated worker to the site gate to meet the response
team (medical, fire, etc.) and ensure that they have a safe access path to the accident
scene.
12. The site supervisor assembles the emergency rescue team at the accident site as
quickly as possible to determine the best rescue procedure for the situation.
Rescue Procedures
If the fallen worker is suspended near a work area and can be safely reached from the platform
floor below or the area from which they fell, use the following procedure.
5. Once the worker has been brought to a safe location, administer first aid and treat the
person for suspension trauma or any other injury.
6. Arrange transportation to hospital if required.
Post-Rescue Procedure
All non-affected workers should remain in the designated safe gathering zone until the site
supervisor notifies them to do otherwise.
The site supervisor and TPL health and safety representative should,
• Begin the accident investigation; an initial report should be issued to CGPL within 24
hours.
• Quarantine all fall-arrest equipment that may have been subjected to fall fatigue effects
and/or shock loading for further investigation.
• Secure the area especially ensuring that an accident scene is not disturbed where a fatal
or critical injury has occurred.
• Collate and gather together the JSA and methods statements are determine whether or
not the job site-specific rescue and evacuation plans were followed as designed.
• Record modifications or additions to the plans that the rescue team deems necessary.
• Record all documented communications with the doctor, ambulance, fire, police and
other contractors involved.
• Record all documented statements from employees, witnesses and others.
• Save all photographs of the incident.
• Record all key information such as dates, time, weather, general site conditions, and
specific accident locales including sketches of the immediate incident area, complete
with measurements if applicable.
Remember:
a) Stop all work
b) First aid is to be given to the patient whilst waiting assistance.
c) If able to walk send to bottom with someone in attendance to seek medical assistance.

d) If unable to walk send one person and the passenger cage down to meet the medical
team.
e) Escort the Nurse/doctor to top, assisting in carrying the medical equipment, briefing
them of any changes in the patient.
f) If the person can bear his own weight send him and the medical down to the ambulance.
g) If he is unable to stand and a stretcher is required to carry him (procedure for patient and
stretcher evacuation?)
Supervisors will be available at top and bottom of the chimney and are to coordinate any
emergency and normal shift management.
Workers will only be employed if they pass a thorough medical check-up prior to being allowed
to work at the top.
For continuous in work, need regular rest to minimize fatigue of personnel.
Power Outage
CGPL has arranged for power grid supply in case of an outage or failure of the grid, TPL has
provided a 125 KV diesel generator as standby.
Regularly the generation set is to run for 30 minutes based on the inspection of
electrician.
DG set is tested for running in case of power failure.
Only in the event of an emergency evacuation of all personnel due to fire or other catastrophic
events, we may use winches with buckets as well as the passenger cage.
Therefore all winches and buckets must be inspected and tested as if personnel carrying
equipment and in line with the Factories Act.
• Local police, fire or bomb disposal or CGPL authorities will be immediately notified.
• The project or office shall be evacuated immediately.
• Evacuation will be made of all personnel on the project or in the office.
• A personnel count should be made to ensure that all are present and accounted for.
• Do not allow anyone except authorized personnel to re-enter the area.
• If necessary to stop or detour traffic away from the affected area, utilize local
police or flagman.
• A search of the premises will be made by the appropriate authorities. No
company supervisory personnel or other employees shall be involved in the
search.
• Notify other business people or residents who may be endangered by the
threat.

Enter the premises only after careful evaluation of the circumstances, (to be made by the
company's senior management in consultation with appropriate law enforcement authorities) and
permission of local management.
7.15 Earthquake
If you are inside the office building when an earthquake occurs:
• Stay away from windows, large panels of glass, ceiling tiles, file cabinets,
bookcases, heavy furniture, electrical equipment and appliances.
• Crawl under a desk or sturdy table and hold onto it. If it moves, move with it.
• Stay clear of falling objects and find a place to duck, cover and hold on.
• Stay near an interior wall and protect your head with your arm.
Drivers of all vehicles except for fire trucks, ambulances and other emergency vehicles
shall park their vehicles clear of roadways, with ignition switch turned “Off” and the key
left in the switch. If the person is inside the affected area, the person shall immediately
wear PPE prescribed while proceeding toward to the safest Assembly Point. If the
designated assembly point/area is unsafe or blocked due to the emergency, proceed to
the alternate assembly point.
Move quickly but do not run (except in life threatening conditions) to the assembly
points.
Do not overtake along the route
Do not smoke in the assembly points
Do no return to a work area to collect belongings or for any other reasons until the “all
clear” signal is given
Visitors are under the responsibilities of their hosts and should be accompanied to the
nearest safe assembly points.
In case of thunderstorm, heavy rain and works happening at height such as slip-form
activities, erection activities. All workmen and staffs will be guided by the ERT members,
the ERT team will assess the situation and will give to warning/alarm to the
workmen/staffs at height before the weather condition becoming critical. All work is
stopped with the help of duty shift in-charge of safety officer, all workmen are brought
down safely by using the access arrangements made for coming down from height to
ground and will lead to the nearest assembly point/rest shelter.
Phones and radio channels shall be kept free and used only for emergency related
communication.
Periodic emergency drills will take place quarterly in coordination with CLIENT (Head-Fire &
Safety-CGPL and Risk Manager-CGPL).

Mock Drill will be conducted by taking prior approval with Head Fire & Safety, CGPL.
Drills shall be documented and corrective actions taken to implement any lessons learnt.
TPL will issue and develop a dedicated “HSE Training Program” with details of emergency and
evacuation trainings to be attended by all Site/Project personnel.

distributed by TPL to their respective Supervisors and HSE staff for linkage/coordination
purpose in an emergency situation.
Hand-held “Loud Speakers” will be provided by TPL at each jobsite work location for the use of
the designated Area Supervisor and to assist its HSE staff in spreading the awareness of an
emergency situation.
If temporary absent from the location, the TPL Supervisor shall designate his Foreman for the
prescribed usage of emergency Loud Speaker. This information will be included in Training
session.
Head counts shall be done against manpower reported by relevant Supervisors at each
emergency including mock drills. To facilitate the response to an emergency situation,
HR/Admin will be adopted complete and report daily head counts of all direct field labors.
During the evacuation, these head counts will be used to determine whether any personnel
remain unaccounted.
In the event that a missing person is identified through the head count process at the Assembly
Point(s), the concerned supervisor shall undertake the following steps:
Identify the missing person, his work group team leader
Ascertain the activity of the missing person
Determine the location of the missing person immediately prior to the emergency
evacuation requirement
Notify the Incident Controller (Site Manager)/CGPL Shift In-charge of the missing
person(s).
The Incident Controller will relay information and convey for assistance to client.
Site HSE Manager shall activate the ERT when clear determination of the location and
condition of the missing person is made clear
Communication of the missing person shall also be done to client for further action.
