RE: OSSAI GODSTIME OGECHUKWU; MALE; 25
YEARS.

The above named patient is a patient of ours who presented at the hospital in
December, 2022 on account of Recurrent fever with Right Lower Abdominal pain. He is
a peptic Ulcer patient.

Right of FBC showed absolute neutrophilia with monocytosis.

Abdominopelvic ultrasound showed a hypoechoic and circumscribed area around the

ileocecal junction, suggestive of a chronic Appendicitis with abscess.

Hence it therefore medically recommended that any situation that will place under
strain on the patient should be avoided.

In view of the above ailment, the bearer is therefore advised to be close to his doctor in
case of emergency, kindly use your office in granting the bearer the permission to
relocate on account of his health challenges.
