Quezon City for School Year 2024-2025, may I respectfully request your approval
to retain my scores from the previous open ranking SY 2023-2024, specifically for
the following components or criteria:

By making this request for retention, I understand and agree to the following:

1. I may no longer need to participate in the open evaluation of documents

and/or undergo demonstration teaching/teacher reflection, provided that I
have undergone such processes last year as a first-time applicant of SDO
QC.
2. I still need to comply with the cut-off score of 50 points for my name to be
included in the Comparative Assessment Result - Registry of Qualified
Applicants (CAR – RQA). Inclusion in the CAR-RQA does not guarantee an
appointment since it is based on the identified needs of the Division.
3. I am making this request only to help facilitate the application since there
are hundreds of applications to be processed. However, I may withdraw this
request and therefore agree to undergo the entire recruitment process from
start to finish within the period allotted for the recruitment for Teacher I for
School Year 2024-2025.
