Agricultural Vocational High School had been granted permission by my parent/ guardian to undergo
Work Immersion at DON BOSCO TRAINING CENTER From , 2023 to ________

Further, I hereby voluntarily agree to undergo the said immersion in any actual
demonstration as required by the course Organic Agriculture Production (OAP) NC-II and in order to
acquire actual work experience with the following terms and conditions:

1. I understand that the school, Taguibo Agricultural Vocational High School will not be liable for
any untoward incident that may happen while on my way and to the training site (DON BOSCO
TRAINING CENTER) and in the performance on my duties and functions while under immersion/
training.
2. That I shall abide to the school’s rules and regulations (particularly in health protocol during this
time of pandemic) and shall comply with the imposed requirements for the Work
Immersion/training otherwise, I shall be excluded from further participation.
3. That I shall exercise care and diligence in any task assigned to me.
4. That I shall be made answerable in all liabilities for damages to property or injury to third person
which may be occasioned by my intentional or negligent acts while in the course of my training.
