1 Operation boat transfer
- Due to the bad weather condition, extra percaution is needed during boat transfer
- Use two hands to hold the rope during boat transfer
- Life jacket need to be wear

2 Safety briefing for all SS3's arriver and leaver

3 Gangway crossing
- Use two hands to hold rope when crossing gangway
- Gangway id card need to be place on and collect upon return
- Life jacket need to be wear when crossing gangway

4 SS3 Barge
- Escape routes displayed around the barge
- Emergency phone number was displayed in the operation room
- Observed clean desk policy was practiced in the operation room/kitchen
- Emergency alarm was practiced. Everyone need to collect the Key Card and gather at the mustering station with life jacket.
- Holding hand rail when using stair case

5 Barton Platform
- Ear plug need to be wear due to high decibel (> 90dB)
- No cell phone is allow on platform.
- Beware of sea snake
- Safety boot is not allow to put infront of door as this may block the door for opening
- No smoking on platform
- Trip hazard

6 Coil Tubing Sand Cleanup


- Coil tubing units were checked on August/November 2008
- Observed the Schlumberger contractors were using proper glove and mask during the mixing of chemical for sand clean up
- Nitrogen was running out during sand clean up at 4200ft. The remaining 200ft were clean just using gel
- Observed how the Schlumberger & Smith Services change the BHA with miller/nozzer for sand clean up
- Observed how the coil tubing unit look like
- Observed red tape was put around the operation area
- Observed gel spilled during mixing and cleaning up the area was done
- Observed some part of KOIV and sand compaction were retrieved out udrinf sand clean up using miller
