 CURRENT HEALTH STATUS

- According to the SO the patient is already diagnosed with a gallstone and then decline the
operation until it becomes worst and then there are other complications like in the heart and her
heart beat is fast but her blood pressure is only 70/50.

 PAST MEDICAL HISTORY

- As verbalized by the SO that was the only past medical history of her mother the gallstone.

 FAMILY HISTORY
- According to the SO the sister’s of her mother has ulcer and heart complications but there
are no surgery.

 HEALTH PERCEPTION AND HEALTH MANAGEMENT

- According to the SO her mother is already in pain that makes her uncomfortable until she
cannot endure it that is why they decided to go the hospital.

 NUTRITION AND METABOLIC PATTERN

- Before the patient eats a lot of vegetables and meat but now she only eat cerelac (vegeteble),
and porridge.

 ELIMINATION PATTERN
- Her output is normal before admission and during it’s just that right she is wearing diaper.

 ACTIVITY EXERCISE
- According to the SO the patient is the one who does the laundry of her grandchildren before
she was confine but right now she’s just lying down and no exercise at all.

 SLEEP REST PATTERN

- According to the SO before confine and during she sleep like she always does like normally.

 COGNITIVE-PERCEPTUAL PATTERN
- According to the SO the patient had already ultrasound and x-ray and she stated that the
gallstone is already big unlike before.

 ROLE RELATIONSHIP PATTERN

- The patient is friendly towards others but right now she can’t talk to me unlike before.
